Midlands Partnership University NHS Foundation Trust (MPFT) is excited to offer an opportunity for a Band 6 Senior Staff Nurse to join our Community Intervention Team within the South East Staffordshire locality, based at Merlin House in Tamworth.

This is an exciting role for a motivated and enthusiastic nurse who is passionate about delivering high-quality, patient-centred care in the community. You will play a key role in supporting patients to remain in their own homes and avoid unnecessary hospital admissions, whilst delivering safe, effective, and responsive care.

Main duties of the job
As a Senior Staff Nurse within the Community Intervention Team, you will be responsible for delivering and coordinating acute and planned nursing interventions to patients in their own homes and residential care settings. Patients supported by the service often have complex health needs requiring advanced clinical assessment, treatment, and ongoing management.

You will be expected to demonstrate a wide range of clinical skills including:

• Administration and monitoring of IV antibiotic therapy.
• Venepuncture and line care.
• Catheter care (TWOC's) and management.
• Assessment and management of long-term conditions.
• Clinical assessment and decision-making.
• Supporting patients with complex physical health needs.
• Coordinating care with multidisciplinary teams and partner organisations.

The successful candidate will be confident working autonomously, whilst also contributing positively to a supportive and dynamic team environment.

Job responsibilities
You will:

• Be a Registered Nurse with current NMC registration.
• Hold a Degree in Nursing or equivalent qualification.
• Hold postgraduate study in a relevant area such as Long-Term Conditions Management, or Advanced Clinical Practice.
• Have experience working within community nursing, acute care, or integrated healthcare services.
• Demonstrate excellent communication and interpersonal skills, with the ability to build effective relationships with patients, carers, and multidisciplinary teams.
• Be confident in working independently and managing a diverse caseload while prioritising workload effectively.
• Possess strong clinical assessment, problem-solving, and decision-making skills.
• Have experience supporting hospital discharge processes and admission avoidance pathways.
• Be committed to delivering safe, effective, and compassionate patient care.
• Demonstrate leadership qualities and a willingness to support, mentor, and develop colleagues.
• Show a commitment to continuous professional development and lifelong learning.

• Post graduate study in relevant subjects
• Mentorship qualification or Practice assessor qualification
• Current Physical Health Assessment qualification or willingness to complete within 12 months of appointment to role.

Experience

• Post qualification courses in long term conditions or relevant subjects. Attendance at relevant groups, e.g. Specialist Interest Groups.
