Description
Band 7- District Nursing - Advanced Nurse Practitioner (ANP)
Location: Helensburgh & Lomond
Hours: Full time - 36 hours per week
An exciting opportunity has arisen within the Helensburgh and Lomond Community Nursing Team for an experienced and motivated Advanced Nurse Practitioner (ANP) to join our service.
Serving a semi-rural population of approximately 26,000 across five GP practices, the post holder will work autonomously and collaboratively within the wider multidisciplinary team to provide advanced clinical assessment, clinical decision-making and prescribing for patients with complex and changing health needs within their own homes and community settings.
The role will have a strong focus on early intervention, prevention and hospital avoidance, supporting the early identification and management of deterioration and enabling patients to remain safely at home wherever clinically appropriate.
The post holder will contribute to the management of frailty and long-term conditions, future care planning, palliative and end-of-life care, and the prevention of avoidable hospital attendance, admission and readmission.
This is a new role within the District Nursing service and is an exciting opportunity for the successful candidate to contribute to the development and evaluation of advanced practice within the team.
The working pattern may be subject to change as the role becomes established, based on identified service need, patient demand and evaluation of the developing service model. Flexibility will therefore be required.
The post holder will work closely with District Nursing colleagues, General Practice and the wider multidisciplinary team, providing advanced clinical expertise and supporting proactive, person-centred care closer to home.
Please refer to the Job Description and Person Specification for the full list of required qualifications, experience, essential criteria and skills necessary for this post.
Applicants must demonstrate that they meet all essential criteria within their application to be considered for shortlisting.
Please note that this post is not eligible for Skilled Worker visa sponsorship. Applicants must therefore already have the appropriate right to work in the UK for the duration of the appointment.
