Occupational Therapist in
Coventry and Warwickshire Partnership Trust to join the
Southwest Community Mental Health Team in Stratford Healthcare, Stratford-upon-Avon. You will be responsible for providing specialist occupational therapy assessment and intervention to adults experiencing severe and enduring mental health difficulties, supporting service users to develop and maintain meaningful occupations, routines, relationships, roles and activities that promote recovery, independence and quality of life. You will undertake holistic occupational assessments, develop collaborative intervention plans and deliver evidence-based occupational therapy interventions within community settings. You will work as an integral member of the multidisciplinary team, providing specialist occupational therapy expertise to assessment, formulation, care planning, risk management and recovery planning, while supporting service users to achieve their individual goals and improve their overall wellbeing and independence.
Main duties of the jobAs an Occupational Therapist you will be responsible for identifying barriers affecting individuals ability to carry out everyday activities and developing practical strategies to address these. You will manage the formulation of individual occupational therapy goals in partnership with service users and deliver tailored programmes designed to improve engagement in self-care, productivity, education, employment, leisure and social activities. You will support individuals to establish sustainable routines, build coping mechanisms, develop motivation and confidence, and strengthen their self-management skills through graded activities and meaningful tasks. You will consider the social, psychological and environmental factors influencing an individuals ability to participate in everyday occupations and adapt your approach accordingly. You will monitor progress against agreed goals, evaluate outcomes and make appropriate changes to treatment plans where required. You will also be responsible for providing clear education, guidance and practical advice to service users, carers and other professionals involved in their support.

Job responsibilities- To assess functional abilities, cognition, executive functioning and sensory needs where clinically appropriate, identifying difficulties that may affect an individuals ability to manage everyday activities safely and effectively.
- To recommend suitable equipment, adaptations and environmental changes, liaising with housing, benefits, education, employment and community services to address practical needs and improve the individuals circumstances.
- To coordinate care where required, contributing specialist occupational therapy input to MDT reviews, CPA processes, clinical formulations and referrals while ensuring effective communication and continuity across services.
- To assess and manage risks associated with functional activities, self-neglect, vulnerability and community environments, escalating safeguarding concerns and following relevant Trust procedures, legislation and risk-management requirements.
- To maintain high standards of professional practice by keeping accurate clinical records, undertaking supervision and appraisal, maintaining HCPC registration and mandatory training, and contributing to audit, research and quality improvement activities.
- To provide clinical guidance and professional support to junior colleagues and students, contributing to their learning and development while supporting the wider multidisciplinary team to understand and apply occupational therapy approaches
Person SpecificationExperienceEssential- Degree or equivalent qualification in Occupational Therapy. Current HCPC registration. Relevant post-registration experience working with adults with mental health needs. Experience of occupational assessment, intervention and multidisciplinary working. Understanding of recovery-focused and person-centred practice, with good communication and risk-management skills. Knowledge of the Mental Health Act, Mental Capacity Act, safeguarding, confidentiality, information governance and relevant clinical information systems
System KnowledgeEssential- Knowledge of NHS clinical systems and electronic patient record systems Mandatory
- Knowledge of safeguarding procedures, information governance, confidentiality and data protection requirements Mandatory
- Knowledge of relevant NHS policies, procedures and clinical governance requirements Mandatory
- Knowledge of local referral pathways and multidisciplinary working within Community Mental Health Teams (CMHTs) Desirable
- Knowledge of relevant mental health legislation, including the Mental Health Act and Mental Capacity Act Mandatory
- Knowledge of risk assessment, risk management and care-planning systems Mandatory
