Main duties of the job
As a Band 7 Specialist Palliative Care clinician, you will provide expert, patient-centered care across hospital settings, supporting complex symptom management, end-of-life planning, and holistic decision-making. You will lead clinical practice, mentor colleagues, and collaborate with our multidisciplinary team including social workers, psychologists, complementary therapy practitioners, and discharge case managers.

You will make autonomous decisions such as initiating advanced symptom control, determining suitability for complex discharge or community support, and advising on ethical or challenging care dilemmas. Your role requires advanced clinical assessment, prescribing where appropriate, and high-level communication with patients, families, and colleagues.

We are looking for someone with palliative care experience--whether in hospice, community, or acute settings--postgraduate palliative care qualifications, and strong leadership skills. Job responsibilities

• To operate autonomously at an advanced level of clinical practice providing research based expertcare and advice to all colleagues and managing a caseload effectively, making the most of availabletime
• To accept referrals from medical/nursing staff both within and external to the Trust, of patients whorequire specialist palliative care input for complex symptom control and complex psychologicalsupport
• To ensure that patients are seen within the recommended time scales and targets
• To utilise specialist, skills, underpinned by in depth theoretical knowledge, to facilitate theassessment, planning and implementation of individualised programmes of care for patientspresenting with a specialist condition and to evaluate their programmes of care.
• To undertake initial holistic assessment of patients including:a) assessment/investigation of symptomsb) evaluation of test resultsc) recommendation of further diagnostic tests
• To respond to telephone enquiries giving advice or taking action as appropriate.

Qualifications
Essential

• Registered Nurse on appropriate part of the NMC register
• Postgraduate qualification relevant to the specialism or equivalent

Desirable

• Advanced Communication skills training or equivalent

• Extensive post-registration experience minimum 4 years and experience in specialism
• Evidence of advanced clinical skills relevant to the role
