Are you a registered nurse looking for a role in District Nursing. We are looking for an experienced and motivated individual who will be responsible for carrying out holistic and autonomous evidenced-based nursing care to patients under the care of the Warrington Out of Hours District Nursing service. You should be able to demonstrate ongoing professional development.

The post holder will provide a high standard of care, utilising clinical skills in the assessment, planning and implementation of pro-active care to a community population with nursing needs referred into the district nursing service. The post holder will work in close collaboration with other professionals/stakeholders within the Warrington footprint.

Main duties of the job
The OOH role focuses on promoting patient safety, health and wellbeing through timely assessment and intervention in response to urgent and priority clinical needs. The postholder undertakes comprehensive assessments of Priority 1 callouts, identifies changing or deteriorating health needs, delivers appropriate nursing interventions and evaluates outcomes. They work autonomously and without direct supervision, demonstrating clinical competence, professional accountability and adherence to NMC standards, while maintaining accurate and contemporaneous records.

The postholder works collaboratively with the wider multidisciplinary team, patients, relatives, carers and relevant health and social care services to ensure safe and effective care. The role does not involve holding a caseload or undertaking planned chronic disease management; instead, it is focused on responding to urgent and unpredictable OOH demand, prioritising patients according to clinical need, managing potentially complex or challenging situations, and escalating concerns appropriately. Effective communication, sound clinical judgement and the ability to work independently are essential to maintaining patient safety and continuity of care across the OOH service.

Job responsibilities
Job Purpose:
The post holder actively supports the District Nursing team by undertaking responsibility for a delegated caseload.

Undertakes the assessment, planning and evaluation of patients care needs.

Liaises with the District Nurse to ensure information sharing in relation to patient care / changes to care plan and the continuity of care and reviewing of complex care packages.

Mentoring, Teaching and Assessing.

Liaises with all health care professionals and other agencies to ensure effective service delivery.

Duties and Responsibilities:

• To promote the health and wellbeing within the family unit and community as a whole.
• Perform comprehensive assessment of patient nursing needs as the named nurse.
• Plan, assess and evaluate programmes of care according to changing health needs under the direction of the District Nurse.
• To support the team leader in the co-ordination of complex and continuing care packages.
• Maintain accurate records in accordance with NMC.
• Attend a multi-disciplinary case conference as the named nurse if appropriate.
• To communicate and collaborate with other members of the PHCT, voluntary and statutory organisations to support individual patient care.
• To establish and maintain effective communication with patients, relatives and carers in complex and potentially challenging situations.
• To competently perform clinical skills related to the post.
• To be professionally responsible for a delegated caseload under the supervision of the District Nurse.
• To assess health needs at individual, practice and community level.
• To undertake chronic disease management reviews using appropriate template.
• To work without direct supervision

Managerial:
To access support from the District Nurse Coordinator or District Nurse within the group for the day-to-day workload in the absence of the teams designated District Nurse.
