Description

Job summary
This exciting Band 5 Intermediate Care Nurse opportunity will be primarily based on Ward B3 at Halton Hospital, supporting the Community Winter Stepdown Ward from 1 December 2026 to 31 March 2027. The ward has been established to support system winter pressures, providing rehabilitation, assessment and recovery-focused care for patients requiring intermediate care support following an acute hospital stay or as an alternative to hospital admission.

Whilst the role will remain focused on the delivery of high-quality intermediate care throughout the duration of the winter ward, successful candidates will also benefit from the opportunity to gain broader experience across the Community and Dental Directorate following the winter period. Subject to service requirements, posts may be redeployed into a range of community nursing services, including Community District Nursing Teams, Intermediate Care Services, and other Community Staff Nurse roles, offering valuable development opportunities and exposure to diverse community healthcare settings.

This post represents an excellent opportunity for nurses who are passionate about rehabilitation, patient-centred care and supporting patients to regain independence while developing their skills across a wide range of community nursing services.

Main duties of the job
  • Assess, plan, implement and evaluate patient care in line with individual needs and rehabilitation goals.
  • Deliver high-quality, evidence-based nursing care to patients requiring intermediate care and rehabilitation support.
  • Promote patient independence and support recovery through personalised care planning.
  • Work collaboratively with the multidisciplinary team, including therapists, medical staff and social care colleagues, to coordinate patient care and discharge planning.
  • Undertake comprehensive patient assessments, identify risks and escalate concerns appropriately.
  • Administer medications safely and in accordance with Trust policies and professional standards.
  • Maintain accurate, contemporaneous patient records and documentation.
  • Support patients and families with education, health promotion and discharge planning.
  • Participate in ward-based quality improvement initiatives, audits and service development activities.
  • Act as a patient advocate, ensuring care is delivered with dignity, compassion and respect.
  • Supervise and support healthcare assistants, students and junior colleagues where appropriate.
  • Contribute to the creation of a safe clinical environment through effective infection prevention and control, risk management and safeguarding practices.
  • Participate in the ward's seven-day service and work flexibly to meet service demands.
