Job summary
The Frailty team is based at Warrington Hospital assessing and treating patients with frailty across the frailty pathway which includes Frailty Assessment Unit Out-patient Clinic, Emergency Frailty Department, ED in reach and Acute Frailty Same Day Emergency Care.

The team is made up of an experienced multi-disciplinary team including senior advanced practice therapists with an extensive range of experience in frailty care.

The acute frailty pathway supports people living with frailty to self-care and maintain what matters to them most. We provide a truly integrated pathway, collaborating with our health and social care partners to improve patient care and communication across the whole system.

Main duties of the job
Working closely with the experienced multidisciplinary frailty team, primary care, social care, voluntary services, patients and carers, the post holder will coordinate care and provide specialist advice where clinical presentations, communication needs or discharge arrangements are complex across the Frailty Assessment Unit, outpatient clinic, Emergency Department in-reach and Acute Frailty Same Day Emergency Care.

Experience

• Relevant post registration experience in broadly similar area of work.

Desirable

• Band 5/6 Experience in Frailty
• Experience in supervising other staff or students

Qualifications

• Bsc in Occupational therapy
• HCPC registration

• Related courses to Frailty
• Student educator course
• Basic leadership course

skills knowledge and competencies

• knowledge of Frailty care within acute/ front door setting

• attendance on frailty related course
