We employ a dedicated team of Home Visiting Clinicians including Community GPs, Community Matrons and Paramedics who deliver high-quality, holistic care for our most complex patients. Key Responsibilities
  • Conduct face-to-face and telephone consultations, including home visits
  • Deliver a structured and protected weekly planned care home ward rounds in line with the Enhanced Health in Care Homes (EHCH) model, ensuring proactive, personalised care for residents
  • Work autonomously within your professional scope of practice
  • Make appropriate referrals and ensure clinical safety and excellence
