Job summary
The post holder will work in collaboration with service users and carers; ensuring nursing and professional practice is guided by the principles of least restrictive care outcomes, recovery and good clinical practice. Act as an exemplar and professional role-model in embracing core values of dignity, respect, care and compassion.

Main duties of the job
  • Be accountable and responsible for the full assessment of care needs and outcomes for patients presenting with a wide variety of clinical conditions. This will be as an individual and as part of the multi-professional team. This will include decisions to admit or discharge.
  • Provide a comprehensive consultation-liaison service toward teams and individual clinicians, particularly in assisting them to develop plans of care and make difficult decisions in cases where patients are exhibiting complex psychosocial needs and/or challenging behaviours.
  • Undertake risk assessments for service users and be responsible for communicating information about the risk to all relevant services and agencies. To work in partnership with the user, carer and these agencies and have an understanding and awareness of the differences in their values and cultures as well as priorities and objectives and how those differences can affect partnership strategies.
  • Participate fully in quality assurance initiatives, with particular emphasis on a 'patient first' approach.

In order to meet the needs of the Trust you may be required from time to time to work at different locations to your normal place of work. This may mean that you are required to work at any location that fall under Barnet, Camden, Enfield, Haringey or Islington. The Trust reserves the right to require staff to work at such other places or locations as it considers reasonable and necessary on a temporary or permanent basis.
