Job summary
The post holder will be employed to work within an intensive community-based service, coordinating a number of health and social care interventions across a range of needs. This is a newly qualified nursing developmental role.

The Camden and Islington Early Intervention Services (EIS) are well-established, multi-disciplinary teams that serve the culturally diverse communities of Camden and Islington. The teams are dedicated to promoting full symptomatic and social recovery, with a strong emphasis on supporting service users to engage in meaningful education and employment opportunities.

Service Aims:

• To provide early assessment, diagnosis, and treatment of first episode psychosis.
• To deliver person-centred, culturally sensitive care that promotes stability, skill development, and meaningful social roles.
• To support both symptomatic and social recovery through sustained, effective engagement with service users.
• To minimise stigma and reduce the potential for negative experiences in early contact with mental health services.
• To actively involve and support carers through education and collaborative care planning

Main duties of the job
As a Nurse Practitioner, the post holder will be responsible for coordinating a variety of multi-disciplinary interventions as part of each service user's treatment journey. They will act as the primary point of contact for service users, carers, and other professionals involved in the individual's care. This includes:

• Leading referrals to appropriate services and support systems based on team discussions and treatment planning.
• Developing, implementing, and monitoring care plans and discharge plans under the supervision of the Team Manager or designated clinical lead.
• Delivering care in line with NICE guidelines, evidence-based practice, and principles of recovery and reablement.
• Maintaining responsibility for a defined caseload and contributing to the ongoing development of person-centred care and treatment plans for individuals with mental health needs

In order to meet the needs of the Trust you may be required from time to time to work at different locations to your normal place of work. This may mean that you are required to work at any location that fall under Barnet, Camden, Enfield, Haringey or Islington. Job responsibilities
As a member of the Intensive Community Mental Health Team, the post holder will play a key role in coordinating, monitoring, and delivering a range of tailored interventions to meet the individual needs of service users, as identified by the multi-disciplinary team. This includes providing direct care and support as part of a holistic treatment approach.

Qualifications and Registrations
Essential

• NMC Qualification - Newly Qualified Developmental Role
• Current Level 1 Safeguarding Children and Safeguarding Vulnerable Adults certificates or willingness to undertake

Desirable

• Evidence of recent and ongoing academic study
• Level 2 Safeguarding Children or Safeguarding Vulnerable Adults certificate
• Mentorship or recognised teaching and coaching certificate

Skills and Abilities
Essential

• Excellent verbal and written communication skills, Able to work on own initiative and Able to work as part of a team
• Competent using computers and keyboards for entering personally generated data, generating and responding to emails, conducting web searches etc.
• Motivated and enthusiastic
• Demonstrate and understand the recovery philosophy
• Ability to maintain boundaries and form positive therapeutic relationships with service users and carers
• Sound awareness of patient / service user care within the wider context of the organisation
• Demonstrate a clear understanding of the roles within the multi-professional team
• Demonstrate well developed interpersonal skills

Experience and Knowledge
Essential

• Demonstrable experience of caring for people with mental health issues in an in-patient, community or substance misuse care setting
• Demonstrable experience of working in mental health or a substance misuse setting
• Demonstrable experience of working in a multi-partnership approach
• Demonstrate a good understanding of the biopsychosocial factors that could have an impact on an individual's wellbeing and preferred treatment methods
• Experience of or, if appropriate to service needs, be willing to undertake training in de-escalation or breakaway techniques.

Desirable

• Demonstrate a clear understanding of current Mental Health legislation and its application / use in practice or legislation relevant to substance misuse services.
• Practical experience of mentoring or supervising students, other trainees or new staff
• An awareness of current research and evidence-based practice within the setting
• Demonstrate an understanding of the management of the physical health needs of people with mental health or substance addictions problems.
