Job summary
The MACCT is lead jointly by Whittington Health NHS and Haringey GP federation. It is a leading example of this integrated working and one of North London's earliest neighbourhood-based care models. Bringing together professionals from health, social care and the voluntary sector to support adults with frailty or complex needs, MACCT is widely recognised as the model on which the Integrated Neighbourhood Team (INT) can be built, demonstrating what truly joined-up, preventative and person-centred neighbourhood care can achieve.

Colleagues appointed to this role will join an ambitious and inclusive partnership shaping the future of integrated care in Haringey. You will work closely with clinicians, community partners and system leaders to support residents in a more connected, preventative and person-centred way. These posts are funded for one year (until 31/3/27) through neighbourhood test-bed funding, providing an opportunity to build on the MACC model and support the development of a model Integrated Neighbourhood Team (INT).

Main duties of the job

• To collaborate with partner agencies in the development of the local delivery of services provided by the MACCT .
• To deputise for MACCT Services Manager as required.
• In conjunction with senior managers, participate in recruitment, selection and appointment of staff.
• To provide formal practice and performance management supervision to MACCT/NLFT members in accordance with multi-disciplinary supervision policies.
• To provide specialist professional supervision to members of own profession.
• To undertake yearly staff appraisals and develop Individual Development Plans for MACCT/ NLFT Staff Members.
• To build and nurture links between MACCT and Primary Care and other agencies with the NLFT Mental Health Services (Including but not exclusive to; OP CMHT and Memory service).

In order to meet the needs of the Trust you may be required from time to time to work at different locations to your normal place of work. This may mean that you are required to work at any location that fall under Barnet, Camden, Enfield, Haringey or Islington. The Trust reserves the right to require staff to work at such other places or locations as it considers reasonable and necessary on a temporary or permanent basis.

Job responsibilities

• To be the Lead mental health specialist within your MACCT neighbourhood
• To contribute to the triage of the referral and allocation process for new referrals received by the service.
• To provide highly specialist advice complete, complete assessments and the completion of individual care plans, which take account of risk assessments and individuals psychological and physical health needs (frailty).
• To maintain a role as care coordinator of a case load of clients whose range of needs require complex arrangements of care.
• To ensure good caseload management in including that discharges or transfers of care are progressed in a safe and timely manner.
• To implement a variety of psychological interventions for MACCT caseload.
• To advocate for MACCT clients to receive the most appropriate mental health care and navigate them onto the correct pathway (primary or secondary mental health services).
• To maintain associated record keeping.

Person Specification
Qualification and Registration
Essential

• Relevant professional qualification in Nursing, Social Care or Therapy
• Currently registered appropriate professional body.
• Evidence of ongoing CPD in portfolio

Desirable

• Further training within the specialist area of general Mental Health and for the Older Person

Experience
Essential

• Minimum of four years post registration experience including mental health adults and older people
• Experience of working as part of a team and Experience in Complex Care Coordination
• Evidence of effective workforce development and of using systems to ensure effective staff management (supervision, appraisal and ongoing performance management)

Desirable

• Two or more years' experience of working within a multidisciplinary team

Knowledge and Skills
Essential

• Ability to work and communicate effectively with team members of differing professional backgrounds
• To demonstrate an extensive and sound knowledge base in the provision of mental health services and preferred treatment methods
• Good knowledge of and ability to implement care pathways

Desirable

• Evidence of additional interest: adults living with multiple Long-Term Conditions (Including Mental Health)
