The post holder will be involved in delivering a high quality, holistic care pathway for patients with heart failure across the Community and the secondary care interface. The post holder will play a key role in the delivery of National and local priorities for Heart Failure, through cross boundary working to ensure a service that is both convenient and accessible to patients. They will have a key role in delivering Community Heart Failure clinics / classes or support groups as well as the assessment and review of patients in their own homes.
The post holder will aim to reduce inappropriate admissions into hospital or facilitate an early discharge from hospital for patients with heart failure. Cover will be provided in an In-Reach heart failure service (on a rotational basis) assessing and reviewing patients in hospital and liaising with the Cardiologist to ensure an appropriate management plan is in place. As part of this role the post holder will refer to the appropriate community team local to the patient on discharge from hospital.

Person SpecificationQualificationsEssential- Registered nurse (Adult) with current NMC registration.
- Degree in nursing or equivalent experience.
- Evidence of continuing professional development
- Mentorship/practice assessor qualification (or willingness to undertake) or an alternative teaching qualification
Desirable- MSc or working towards MSc in advanced clinical practice, cardiology or related subjects
- Heart failure module (level 7).
- Independent non-medical prescriber qualification
- Physical assessment skills qualification.
- Leadership qualification.
- A teaching qualification
ExperienceEssential- Significant cardiology nursing experience
- Experience managing patients with complex cardiovascular disease
- Experience working within multi-disciplinary teams
- Experience providing patient education and selfmanagement support
- Experience contributing to audit or quality improvement.
- Experience supporting service development.
- Experience supervising junior staff and students
Desirable- Previous heart failure specialist nurse experience.
- Experience managing an autonomous specialist caseload.
- Nurse-led clinic experience
- Independent prescribing experience.
- Experience delivering IV diuretics surfaces
KnowledgeEssential- Extensive post registration experience within cardiology and/or heart failure.
- Advanced knowledge of heart failure pathophysiology, investigation and evidence-based management.
- Knowledge of NICE, ESC and national heart failure guidance
- Excellent clinical assessment and decision-making skills.
- Ability to interpret ECGs, blood results, echocardiography reports and other cardiovascular investigations.
Desirable- Experience working across acute and community settings.
- Experience of virtual consultations and remote patient monitoring
- Knowledge of quality improvement methodology.
