*Please Note the advert may close sooner than the date advertised due to levels of interest, closing of the advert will happen with no notice*Main duties of the job- Provide high-quality, patient-centred care to individuals with a range of medical conditions especially accurately monitor blood glucose levels ,administer insulin and managing sliding scales ,DKA ,HHS.
- Work effectively as part of a multidisciplinary team, liaising with doctors, nurses, and other healthcare professionals to ensure optimal patient outcomes.
- Manage a variety of cases, with a focus on rapid assessment, care, and discharge planning, ensuring that all patients are ready for a 72-hour discharge.
- Monitor patients' conditions, administer medications, and provide treatments as per clinical guidelines.
- Contribute to patient education and support during their short hospital stay, ensuring a smooth transition to discharge.
- Maintain accurate and up-to-date patient records in accordance with Trust policies
