Job summary
The Salford Adult Social Care (ASC) Therapy Team supports people across Salford--including children--to live as independently and safely as possible in their own homes. We provide person-centred assessments and interventions, enabling individuals to maintain independence and reduce risks within their home environment.

We are a forward-thinking and innovative service with a strong reputation for high-quality practice, particularly in moving and handling, specialist equipment, and complex housing adaptations. Many of our recommendations are funded through Disabled Facilities Grants (DFG), and our therapists have access to a wide range of equipment to support outcomes.

This is an exciting opportunity for a Band 6 Occupational Therapist to develop specialist community skills within a supportive and experienced team.

Main duties of the job

• Screen and prioritise new referrals into the service
• Carry out functional and holistic assessments within service users' homes and, where required, in hospital settings to support safe discharge
• Identify risks and provide practical, person-centred solutions to promote independence
• Recommend and implement specialist equipment, moving and handling solutions, and complex housing adaptations
• Complete DFG-related assessments and recommendations
• Work collaboratively with multidisciplinary colleagues, families, and carers
