At Minehead Medical Centre, we are looking for a dedicated Advanced Clinical Practitioner to join our multidisciplinary team working 20 hours per week, delivering high-quality, patient-centred care to our local community.

Main duties of the job
As an Advanced Clinical Practitioner at Minehead Medical Centre, you will provide expert, autonomous assessment, diagnosis, treatment and management for patients presenting with a range of undifferentiated and complex health conditions.

You will act as a first point of contact, managing acute presentations, routine appointments and telephone or remote consultations. You will undertake and interpret diagnostic investigations, initiate appropriate management plans and manage both acute and chronic conditions.

As an Independent Prescriber, you will prescribe and review medications safely and effectively in line with current legislation and clinical guidelines. You will also manage long-term conditions, support patients with self-management and lifestyle modification, and respond appropriately to urgent or emergency situations

Job responsibilities
As an Advanced Clinical Practitioner, you will work both independently and collaboratively within our multidisciplinary clinical team, using your advanced clinical and critical-thinking skills to assess, diagnose, treat and manage patients presenting with a wide range of conditions.

Your day-to-day will include:

• Autonomous Clinical Practice: Assessing, diagnosing, treating and managing patients presenting with undifferentiated and complex health conditions.
• First Contact Care: Managing acute presentations, routine appointments and telephone or remote consultations.
• Prescribing: Safely prescribing and reviewing medication as an Independent Prescriber, in line with current legislation and clinical guidelines.
• Long-Term Condition Management: Supporting patients with acute and chronic conditions, including self-management and lifestyle modification.
• Urgent Care: Recognising and managing urgent or emergency presentations and arranging appropriate referrals where required.
• Collaborative Working: Working closely with the wider multidisciplinary team to deliver coordinated, patient-centred care.
• Clinical Governance: Contributing to audit, clinical governance and quality improvement initiatives to support safe and effective service delivery.

What We're Looking For

• Registration: Current registration with a UK professional body such as the NMC, HCPC or GPhC.
• Advanced Practice Qualification: MSc in Advanced Clinical Practice or an equivalent qualification meeting the national ACP framework.
• Independent Prescribing: Independent/Non-Medical Prescriber qualification.
• Clinical Experience: Significant post-registration clinical experience in primary care or a relevant setting.
• Advanced Clinical Skills: Strong assessment, diagnostic and clinical decision-making capabilities.
• Life Support: Current Life Support qualification(s).
• Teamwork: Ability to practise autonomously while working effectively within a multidisciplinary team.
• Clinical Governance: Good understanding of evidence-based practice, clinical governance and quality improvement.

Qualifications
Essential

• MSc in Advanced Clinical Practice (or equivalent qualification meeting the national ACP framework)
• Independent/Non-Medical Prescriber qualification
• Current Life Support qualification(s)
• Current registration with a relevant UK professional body

Experience
Essential

• Significant post-registration clinical experience in primary care or a relevant setting
• Demonstrable experience of advanced clinical assessment, diagnosis and clinical decision-making
• Experience managing patients with complex and/or long-term conditions
• Experience working autonomously and effectively as part of a multidisciplinary clinical team
• Understanding and experience of clinical governance, evidence-based practice and quality improvement

Desirable

• Experience of supervising, mentoring or teaching clinical colleagues
• Evidence of ongoing continuing professional development (CPD)
• Knowledge of local and national healthcare policies and frameworks relevant to advanced clinical practice
