Are you an experienced Occupational or physiotherapist mental health clinician looking for an opportunity to influence integrated community care and support people to live well at home?

We are excited to be recruiting Senior Mental Health Practitioners to join our Community Therapy Services across the South, Northeast and City localities of Oxfordshire.

We are looking for a senior clinician with a mental health background, who is enthusiastic to develop their mental health and physical health clinical practice within innovative and supportive multidisciplinary teams, treating patients in their own homes.

The Community Therapy Service aims to provide high-quality integrated therapy services working as a key member of the community MDT. We work closely with other multidisciplinary team members, social services and voluntary organisations for adults with a wide range of physical and mental health conditions.

This role will involve the supervision and training of students, junior colleagues and be involved in teaching of multidisciplinary staff. We have excellent CPD opportunities available including CBT training and Masters modules such as frailty management.

Applicants must have a full valid driving licence and have access to a vehicle for work. Adjustments can be considered in line with the Equality Act 2010 where required.

Main duties of the job

• Deliver specialist mental health assessment, rehabilitation and intervention within community settings.
• Support patients to achieve their recovery and rehabilitation goals, promoting independence and wellbeing.
• Undertake physical care assessment and treatment within scope of own professional practice
• Provide specialist mental health advice, guidance and support to Community Therapy Service colleagues.
• Work collaboratively with Community Mental Health Teams, primary care, social care services, and partner agencies.
• Support effective risk management, clinical decision-making, and care planning.
• Contribute to service improvement and the ongoing development of integrated community care pathways.

Qualifications
Essential

• Occupational therapy/Physiotherapy degree or equivalent
• HCPC registration

Desirable

• Experience in community mental health, rehabilitation or integrated care services.

Experience

• Experience supporting people with co-existing physical and mental health needs.
• Experience of mental health and risk assessment.

Desirable

• Good knowledge of rehab & community services

Skills
Essential

• Excellent written and verbal communication skills
• Good knowledge and skills in elderly care/rehabilitation and have specific clinical expertise in elderly care and rehabilitation

Desirable

• Experience delivering training, education or reflective practice sessions.
