Job summary
The Peritoneal Dialysis (PD) team have a unique opportunity for a band 6 PD Specialist Nurse to join our established team in the PD unit. Part-time post . You would be based at the Churchill Hospital and work within the community, visiting patients at home and across sites OUH (John Radcliffe, NOC).

You would work in a small team, work independently and be flexible. The PD unit is a nurse led service.

You would be responsible for teaching patients how to be self-caring on peritoneal dialysis (adult learning theories), reviewing patients in their home ensuring that their treatment is going well and managing complications.

If you want to make a difference to patients with a long-term condition and their families and enjoy a sense of achievement this is the ideal role for you.

The successful candidate will be treating and supporting patients and their family to manage their long-term condition, thus good assessment skills are vital. Some of the work is also carried out over the phone therefore excellent communication skills are a necessity.

You must be a car driver and have access to a car for work.

Support for Hospitalised Patients
Support is provided to peritoneal dialysis patients who are admitted to hospital, specifically at the John Radcliffe (JR) and Nuffield Orthopaedic Centre (NOC). Additionally, ongoing support is extended to patients on the renal and transplant ward.

Quality Improvement and Audit Participation
The role involves active engagement in quality improvement projects related to patient care and treatment processes. Furthermore, participation in the audit process, with a particular focus on monitoring infection rates, forms a key part of the responsibilities.
