Band 7 Prison Services Clinical Pharmacist to join our integrated healthcare team at
HMP Guys Marsh. You will lead medicines optimisation for patients with long-term and acute conditions, manage repeat prescribing, and deliver structured medication reviews and minor ailment clinics. Working closely with GPs, nurses, pharmacy colleagues, mental health and substance misuse teams, you will support the safe, effective and patient-centred use of medicines within a secure environment. Main duties of the job- Coordinate and deliver structured medication review and minor ailment clinics.
- Manage repeat prescribing and reauthorisation through SystmOne.
- Provide medicines advice, adherence support and clinical reviews.
- Support patients with long-term conditions and high-risk medicines.
- Make evidence-based recommendations to prescribers.
- Support medicines governance, formulary management and clinical audits.
- Analyse medicines data and identify risks and areas for improvement.
- Provide education and training to pharmacy and healthcare colleagues.
- Undertake clinical screening and final accuracy checks when working in the dispensary.

Job descriptionJob responsibilitiesYou will work autonomously and as part of a multidisciplinary team to identify patients at risk of medicine-related harm, optimise treatment and reduce avoidable hospital admissions. You will develop and maintain repeat prescribing procedures and searches within SystmOne, support long-term condition clinics and provide specialist pharmaceutical advice to healthcare professionals and patients. The role includes contributing to clinical governance and medicines management committees, developing policies and procedures, completing audits, supporting public health initiatives and implementing improvements arising from national guidance and medicines safety alerts. You must hold a
Masters degree in Pharmacy or equivalent and be registered with the
General Pharmaceutical Council. You will have relevant clinical pharmacy experience, strong clinical reasoning and communication skills, and a commitment to continuous professional development. Experience within prison, GP, hospital, community, mental health or substance misuse services would be advantageous. The successful candidate will be required to complete an enhanced DBS check and prison security vetting.
Person SpecificationQualifications and Professional RegistrationEssential- Master's degree in Pharmacy or equivalent.
- Current registration with the General Pharmaceutical Council.
- Diploma in Clinical Pharmacy, General Pharmacy Practice or equivalent, or currently working towards this.
- Evidence of continuing professional development.
Relevant Clinical Pharmacy ExperienceEssential- At least three years' experience delivering clinical pharmacy services.
- Experience in a UK prison, hospital or community pharmacy setting.
- Experience undertaking medication histories, structured medication reviews and medicines optimisation.
- Experience working within multidisciplinary teams and providing prescribing advice.
Clinical Knowledge and SkillsEssential- Recent clinical practice in a GP, hospital, or prison setting.
- Strong clinical reasoning and ability to interpret evidence and make safe recommendations.
Desirable- IT literacy; SystmOne experience is desirable
