Community Night Nurse
As a Community Night Nurse at Partnering Health Group (PHL), working as part of the team, your role is crucial in providing an efficient and safe integrated urgent care service to our patients, delivered with a caring and respectful disposition.

Your role is accountable for assessing callers with a wide spectrum of complex clinical conditions, providing safe and effective advice and information, including home care management and onward referrals as appropriate. Ensuring the delivery of a high standard of care and respect in providing quality, evidence-based health care to meet the immediate needs of the patient.

As an experienced Nurse, you will be part of the clinical and operational team, providing clinical oversight of patients within the service to support the safety of the service and mitigate risk. You will be experienced in working independently in Community Nursing Teams, and be accountable for delivering safe, clinical decision-making, and expert care to a high standard. You will form links and develop relationships with internal and external stakeholders, including secondary care and Primary Care, as well as those working in the broader healthcare community, acting within your professional boundaries at all times.

Key Responsibilities
  • To provide safe and effective health assessments and treatments for patients in their own homes
  • To provide safe and effective telephone advice, health assessments and information to the general public
  • To provide holistic patient care within the patient’s home setting from assessment of the presenting condition through to diagnosis and initial treatment, including the administration of medication within agreed clinical guidelines and protocols
  • To provide safe and effective direct clinical assessment and provide the correct ongoing treatment for patients who are referred to the service by SCAS 111
  • To work closely with the medical practitioners in the provision of quality, evidence-based, and cost-effective care to patients within their own homes
  • Undertake effective multidisciplinary liaison across the primary/secondary care interface and the PHL Clinical and Operational Team
  • Support junior team members by offering educational and clinical supervision opportunities and guidance through clinical discussions

About You
Essential
  • 3 years’ experience working as a community nurse
  • At least 4 years post-qualification experience
  • NMC/HCPC Registered
  • BSc Level qualification in Nursing
  • Experience working in a multi-disciplinary team
  • Good organisational skills and the ability to manage and prioritise own workload
  • Excellent communication and interpersonal skills
  • Clear understanding of professional responsibilities and accountability
  • Be able to clear an enhanced DBS check
  • Must be authorised to work in the UK
  • Level 3 safeguarding of adults and children
  • A Full Clean Driving License is required

Desirable
  • Experience with the Adastra and CHIE Clinical IT system
  • Degree level nurse practitioner
  • Independent and supplementary prescribing qualification
