Job summaryWe are seeking an experienced Band 6 Practice Nurse to join our warm, welcoming and supportive team at Pontardawe Primary Care Centre for 30 hours per week.
The initial priority for this role is delivering chronic disease reviews, with a particular focus on diabetes. Previous general practice nursing experience, including independently running chronic disease clinics and undertaking clinical audits, is essential. Strong wound-care skills and experience delivering routine and seasonal vaccinations are also required.
The role will develop in stages, with an initial emphasis on maintaining chronic disease services and continuity of care. Experience in baby clinics and cervical screening is desirable but not essential.
We offer a friendly working environment where colleagues support one another, professional expertise is valued and high standards of patient care are a shared responsibility.
Main duties of the jobThe successful candidate will work independently within their clinical competence, supported by our wider clinical team. Responsibilities will include:
- Chronic disease reviews: Running structured clinics, particularly for patients with diabetes, including assessment, monitoring, patient education, care planning and appropriate escalation to other clinicians. This role will require a main focus on diabetes clinics.
- Clinical audit: Undertaking audits, identifying opportunities for improvement and contributing to changes that strengthen patient care and clinical outcomes.
- Wound care: Assessing and managing acute and chronic wounds, including postoperative wounds, and recognising when further clinical advice or referral is needed.
- Hypertension and blood pressure monitoring: Undertaking hypertension reviews and ambulatory blood pressure monitoring, with treatment and medication decisions supported by the wider clinical team.
- Vaccinations: Delivering routine and seasonal immunisations, including influenza, COVID-19, shingles and pneumococcal vaccinations, within the appropriate training and authorisation arrangements.
- General practice nursing: Providing health promotion, preventive care and other routine nursing interventions, including prescribed B12 injections.
Baby clinics and cervical screening may form part of the wider role where the successful candidate has the relevant training and competence.
