Job summary
Port View Surgery is looking for a dedicated and enthusiastic Practice Nurse to join our friendly and supportive team. This is an exciting opportunity to work in a well-established, forward-thinking practice that is committed to delivering high-quality, personalised care to our community.

As a valued member of our clinical team, you will play a key role in supporting patients while contributing to the wider delivery of excellent primary care services.

Main duties of the job
The Practice Nurse will deliver a comprehensive range of clinical services, including the assessment, treatment, screening, monitoring and health promotion of patients. The role includes the management of long-term conditions, particularly asthma and COPD and the delivery of treatment room services such as immunisations, wound care, cervical screening, venepuncture and other clinical procedures. The post holder will work collaboratively within the multidisciplinary team to promote patient wellbeing, improve health outcomes and support the delivery of safe, effective and patient-centred care.

Job responsibilities
Chronic Disease Management
Adult/Paediatric Asthma
  • Conduct asthma reviews in line with current local and national guidelines.
  • Assess asthma control and develop personalised asthma action plans.
  • Perform and interpret peak flow monitoring and spirometry (where trained and competent).
  • Review inhaler technique and promote adherence to treatment.
  • Identify patients requiring escalation of care and refer appropriately.

COPD
  • Undertake COPD annual reviews and monitoring.
  • Perform and interpret spirometry in accordance with competency requirements.
  • Assess symptom control and exacerbation history.
  • Provide smoking cessation support and health education.
  • Review medication, inhaler technique and self-management plans.

Women's Health/ Hormone Replacement Therapy (HRT) (Desirable)
  • Support the management and monitoring of patients receiving HRT.
  • Carry out reviews and provide health education regarding menopause management.
  • Monitor treatment effectiveness and identify any concerns requiring GP review.

Family Planning and Sexual Health (Desirable)
  • Provide contraception advice and health education.
  • Administer contraceptive injections and other contraceptive services within competence.
  • Support patients in making informed choices regarding contraception.
  • Refer patients to specialist services when necessary.
  • Promote sexual health and safeguarding awareness.

Treatment Room Duties
  • Wound assessment, care and dressing management.
  • Removal of sutures and clips.
  • Venepuncture and specimen collection.
  • ECG recording.
  • Blood pressure monitoring.
  • NHS Health Checks.
  • Immunisations and vaccinations in accordance with national programmes and Patient Group Directions (PGDs).
  • Administer prescribed injections and medications within scope of practice.
  • Infection prevention and control procedures.
  • Chaperoning when required.

Health Promotion
  • Deliver lifestyle advice relating to smoking cessation, weight management, exercise, alcohol consumption and healthy living.
  • Support patients to manage long-term conditions effectively.
  • Participate in health promotion campaigns and screening programmes.

Clinical Governance
  • Maintain accurate and contemporaneous patient records using the clinical system.
  • Work within NMC Code of Professional Conduct and professional standards.
  • Participate in clinical audits and quality improvement activities.
  • Report incidents, near misses and safeguarding concerns appropriately.
  • Maintain confidentiality and information governance standards.

Team Working
  • Work collaboratively with GPs, nursing colleagues, healthcare assistants, pharmacists and the wider multidisciplinary team.
  • Participate in practice meetings and training events.
  • Support the development of junior staff and healthcare assistants where appropriate.

Professional Requirements
  • Registered Nurse with current NMC registration.
  • Evidence of continuing professional development.
  • Competence in chronic disease management, including asthma and COPD.
  • Training in family planning, contraception and women's health (desirable).
  • Competence in spirometry (desirable).
  • Knowledge of current primary care guidelines and best practice.

Person Specification
Qualifications
Essential
  • Registered Nurse with current NMC registration.

Desirable
  • Training in family planning, contraception and women's health.
  • Competence in spirometry.

Experience
Essential
  • Experience working in primary care or general practice.
  • Strong communication and interpersonal skills.
  • Competence in chronic disease, with emphasis on asthma and COPD management.
  • Knowledge of current primary care guidelines and best practice.
  • Evidence of continuing professional development.

Desirable
  • Knowledge of EMIS Web
