Job summary
This is a new role created to support the multidisciplinary team at Powys Living Well Service deliver a biopsychosocial approach to the management of long term conditions specifically: persistent pain, chronic fatigue/ME, long covid and weight management issues. People referred to the service will often be experiencing multimorbidity and have complex needs. There will be a focus on reducing unwarranted polypharmacy and ensuring that any medications taken are used in the most effective way.

The ability to speak Welsh is desirable for this post; Welsh and/or English speakers are equally welcome to apply.

Main duties of the job
The Pharmacist role will be a key contributor to the Powys Living Well Service providing specialist expertise around medicines, building on the existing multidisciplinary team (MDT) members clinical expertise and supporting primary care colleagues and service users. Managing a clinical case load including agreed prescribing responsibilities.Providing expert advice on medicines and therapeutics, including specialist pharmaceutical advice and legislative guidance for the Living Well Service.Leading on policy and guideline development around medicines for the management of long-term conditions. Updating health care practitioners on changes in clinical practice; guidance and national policy around medicines within the therapeutic areas covered by the service.Supporting individuals who would benefit from clinical pharmacist input, including medication reviews, optimising medicines use and identifying opportunities for deprescribing and encouraging self-management.Contribute to teaching, training and supervision of other staff within the service.Undertaking research, audit and service development will be an integral part of service delivery.

Qualifications and Knowledge
Essential

• Registered Pharmacist with the General Pharmaceutical Council (GPhC)
• Master's degree (MPharm) or equivalent BPharm or BSc in Pharmacy
• Postgraduate Diploma in Clinical Pharmacy or Therapeutics or equivalent
• Independent prescriber
• Excellent understanding of pharmacy/medicines standards, legal requirements and guidelines

Desirable

• Recognised leadership or managerial development course, or equivalent experience
• Membership of the Royal Pharmaceutical Society
• Recognised Medicines Information training and experience

Experience
Essential

• Significant recent experience of working as a clinical pharmacist
• Working directly with clinicians and within a multidisciplinary team

Desirable

• Previous experience in undertaking audit and research

Skills and Attributes
Essential

• Good written and verbal communication skills, with an ability to communicate information clearly, accurately and concisely
• Manage professionally challenging conversations and situations
• Analyse, appraise and interpret highly complex data and evidence where information may be limited and open to differing opinions to make an informed recommendation about medication therapy

Desirable

• Some ability to speak, read and/or write Welsh, or an eagerness to learn

Essential

• Ability to travel throughout Powys to sites and GP Practices and to off-site meetings within Wales or England
