Job summaryThe Colne Practice, located in Rickmansworth, is seeking a qualified clinical pharmacist to join the team for three days per week. Ideal working days will be Monday to Wednesday (negotiable) working 9am to 5pm.
Main duties of the job- Clinically assessing and treating patients
- Providing clinical advice and expertise on medication
- Providing specialist expertise in the use of medicines and medicine information (e.g. side-effects, dosage)
- Providing pharmaceutical medication review and health education advice to patients, by face-to-face, telephone or notes-based review, as agreed with the practice
- Ability to plan, manage, monitor, advise and review general care plans for patients with long-term conditions
- Contributing to public health campaigns, including flu vaccinations, childhood immunisation and adult immunisation programmes
- Processing specialist treatment and hospital medication discharge letters
- Monitoring patients with respiratory conditions
- Establishing ongoing professional relationships with individual patients
About usThe Colne Practice is a friendly GP practice with a list size of approximately 10,500 patients. Our team comprises 7 GP partners, 2 practice nurses, 1 HCA, 2 clinical pharmacists and reception and admin teams. We have on site staff parking.

Person SpecificationYou will be a qualified pharmacist with strong experience within primary care and have the following skills and qualifications:Essential- Completion of a degree in pharmacy and registration with the General Pharmaceutical Council
- Qualified independent prescriber
- Member of the Royal Pharmaceutical Society (RPS)
- Awareness of the breadth of common acute and long-term conditions that are likely to be seen in a general medical practice
- Demonstrate general pharmaceutical skills and knowledge in core areas
