Job summary
EAST:MK Primary Care Network has exciting opportunity to welcome a general medical practitioner into our practice teams. The role is offered on sessional basis of between 4 to 6 sessions per week, between Monday and Saturday, on a rotation basis across the 4 surgeries that make up the PCN: Ashfield Medical Centre, CMK Medical Centre, Milton Keynes Village Practice and The Grove Surgery.

Main duties of the job
  • Deliver high quality clinical care to patients through face-to-face/online/phone consultations, home visits, assessments, diagnoses, treatment plans. Manage acute & chronic conditions, safe prescribing, referrals & prescription reviews, maintain accurate clinical records
  • Deliver care plans for chronic conditions
  • Promote immunisations, screenings & lifestyle interventions. Focus on early intervention, prevention & education
  • Refer patients to specialist services & coordinate care across primary, secondary & tertiary sectors. Collaborate with multi-disciplinary teams
  • Engage in health promotion initiatives. Refer patients to relevant agencies & contribute to population health strategies aimed at reducing health inequalities
  • Participate in public health initiatives to address social health determinants & inequalities. Collaborate with local services & community organisations to improve patient well-being
  • Take part in audits, research & quality improvement projects. Ensure adherence to appraisal & revalidation processes, promoting CPD. Contribute to reviews of significant events & near-misses using structured approaches like root cause analysis. Foster a culture of collaborative working & continuous improvement across the PCN
  • Adhere to the highest personal & professional standards as set out by the GMC & put individual patient care as your priority
  • Work with PCN manager, practice managers & clinical director to support the development, delivery & review of the surgery action plan

About us
East:MK PCN covers a total population of approximately 56,000 patients, across its 4 practices in Milton Keynes: Ashfield Medical Centre, CMK Medical Centre, Milton Keynes Village Practice and the Grove Surgery.

We work collaboratively with colleagues in the NHS, public and community services to make as much of an impact to improving patient access and care. Person Specification
We are keen to hear from medical practitioners, looking to expand your contribution to healthcare and personal job satisfaction. All applicants need to be committed to improving health and putting patients first. You must be eager to undertake new ways of working and delivering healthcare, committing to undertaking the professional development required to adapt to changing circumstances. To fulfil the job responsibilities the post-holder is required to:
  • Maintain high standards of care, following relevant codes of practice.
  • Protect confidentiality of personal information and comply with data protection regulations.
  • Ensure health and safety compliance, attend mandatory training, and participate in appraisal processes.
  • Comply with infection control policies and safeguard the welfare of vulnerable individuals.
  • Adhere to equal opportunities and human rights legislation, avoiding discrimination.
  • Familiarise with and follow locally agreed policies and procedures, accessible via the trust intranet.

Essential qualifications, skills, and experience
  • A GP whose name is included in the General Practitioner Register (GPR).
  • Included in the list of medical practitioners maintained and published by NHS England in accordance with section 91 of the NHS Act 2006.
  • Special interest in gynaecology/women's health, paediatrics etc. is preferred.
  • Highly developed interpersonal skills and ability to lead key meetings.
  • Excellent communication skills- written and verbal.
  • Able to travel between sites.
