Clinical Lead for Procare Adult Community Nursing East Waverley

Job summary

We are seeking an inspiring Clinical Lead to join our Adult Community Nursing service to cover maternity leave. You will provide visible leadership across Guildford and Waverley, ensuring safe, high-quality, and responsive care. Covering 12 months from Nov 2026.

The post holder is responsible for the delivery of a safe and high quality community nursing service in a defined locality through the effective line management of the District Nursing and additional service lines teams including the recruitment, budget, appraisals, continual professional development, key performance indicators, compliance, governance, and HR matters.

Main duties of the job

This role combines strategic, operational, and clinical leadership across multiple services:
  • District Nursing
  • Urgent Community Response
  • Night Response
  • Phlebotomy

As part of the Senior Leadership Team, you will:
  • Drive service excellence and innovation
  • Lead workforce development and service transformation
  • Work collaboratively with neighbourhood teams and system partners

We are looking for someone who is:
  • A registered nurse with strong community nursing experience
  • A confident and motivating leader
  • Skilled in communication, problem-solving, and partnership working
  • Passionate about high-quality patient care and population health
  • Able to balance leadership with hands-on clinical practice
  • Ready to take a step into a leadership role

The Procare Chief Operating Officer is a Nurse by background, who has a real passion for all aspects of Community working. Our board is clinically led with a Director of Nursing and GPs who have vast community experience, ensuring that Procare has the expertise and knowledge base needed to develop sustainable integrated services.

Job responsibilities

To ensure the provision of an accessible, responsive, high quality, safe, efficient, and clinically effective locality based adult community nursing service that meets the needs of patients and carers, and is aligned to the needs of the local population

To provide highly visible clinical, professional, and transformational leadership to local adult community nursing teams, professional partners, and stakeholders within each locality area and across the Guildford and Waverley Integrated Care Partnership (ICP)

To work as part of the senior leadership team and triumvirate, including a GP Clinical Directors, Integrated Neighbourhood Teams and Adult Community Health Services area to ensure optimisation of resources, sharing of good practice, driving forward innovation, implementation of service improvement and shared learning

To form effective, sustainable, and good working relationships with local GPs, Practice Nurse, Social Care, Mental Health, and other professional partners

To work with other Clinical Leads, Deputy Head of Nursing, and the senior leadership team to deliver the adult community nursing strategy, NHS and Company values and behaviours

To work clinically 20% of the week

Participate in the manager on call rota as required.

Manage and lead a specific portfolio of services as agreed locally to ensure Procare Adult Community Nursing is delivered across Guildford and Waverley

Manage initial patient complaints and escalate as necessary.

Locally agreed portfolio to cover specific areas such as Health and Wellbeing, Primary Care, Unscheduled Care and Night Response.

Person Specification

Qualifications

Essential
  • Registered Level 1 Nurse with current Nursing and Midwifery Council registration
  • Educated to level 6 / degree level in relevant area
  • Evidence of level 7 / masters level study, or equivalent experience
  • Completion of DN SPQ
  • Teaching / Assessing / Mentorship qualification
  • Clinically competent in areas relevant to adult community nursing as 40% of this post is clinical

Desirable
  • Management / Leadership course
  • Project management qualification
