This unique position offers you the opportunity to work as part of a highly skilled community workforce who are the cornerstone of safe patient care. You will work collaboratively with the multi-disciplinary team to visit patients in their own homes and care homes to avoid unnecessary hospital admissions. We have lots of training and development pathways at every level through from band 3 to band 8. We provide a preceptorship program to support you in your role and ongoing training and development.

We use electronic clinical records through EMIS and a work IPad and IPhone will be provided.

This role has been identified as eligible for the joining incentive of a £2,000.00 phased payment. You will receive a joining incentive payment of £500 after 3 months' service, a further joining incentive payment of £500 after 6 months' service with a final payment of £1000 after a full 12 month's services with Procare Community Services.

Main duties of the job
We are seeking a highly motivated and ambitious District Nurse to join the wider Community Nursing Team which includes Community Matrons, District Nurses, Registered Nurses, Nursing Associates and Healthcare Assistants.

You will undertake clinical skills including venepuncture, wound care, catheterisation and end of life including syringe drivers.

You will manage a caseload of patients attached to 1 or 2 GP surgeries. The role will include caseload management of patients with a range of long-term conditions and multiple pathologies. You will provide holistic assessment of health needs, relevant to nursing care and offer health promotion. You will need to have a District Nursing Qualification and experience of managing a team. You will act as a role model, teaching and supporting staff and students in the community.

Post holders will be available to work Monday to Sunday between 8am and 8pm. A full UK driving license and vehicle for business use if required for this post, however reasonable adjustments will be made for disabled individuals in line with the Equality Act 2010.

Job responsibilities

• Maintains strong working relationships and communicates effectively with GPs, practice nurses, Social Services, Mental Health, acute / community hospital colleagues, Borough Council staff and the local voluntary sector - particularly for patients receiving shared care to ensure an effective flow of patient information to ensure high quality care.
• Attends all Gold Standard Framework and MDT meetings
• Ensures regular meetings are in place with GP and Practice Nurse colleagues

Qualifications
Essential

• Registered Level 1 Nurse with current Nursing and Midwifery Council registration
• District Nursing Specialist Practice Qualification / diploma / degree
• Assessor / NMC Supervisor / Mentorship

Knowledge, Skills and Experience
Essential

• Advanced range of clinical skills and ability to undertake full assessments and physical assessment of patients including those with multiple pathology and complex health and social needs
• Experience prioritising, delegating care and allocating visits in accordance with patient clinical need and individual competencies
