Job summary
An exciting opportunity has arisen for an enthusiastic and motivated Pharmacist to join the Pharmacy Team at Provide. This diverse and rewarding role offers the chance to influence patient care across a wide range of community health services, driving high standards of medicines optimisation, patient safety, quality improvement, and clinical governance.

As a Community Health Services Pharmacist, you will work collaboratively with multidisciplinary teams to ensure the safe, effective, and evidence-based use of medicines across community services. You will play a key role in shaping medicines-related pathways, supporting innovative service developments, and providing clinical advice to healthcare professionals.

The role offers a unique opportunity to lead and influence key strategic workstreams, including Non-Medical Prescribing, Sexual Health Services, medicines governance, and patient safety programmes. You will work closely with clinical teams, operational leads, and senior stakeholders to support service transformation and improve outcomes for patients receiving care in community settings.

This position is ideal for a pharmacist who enjoys working autonomously, leading change, and making a tangible impact on patient care while contributing to the continued development of high-quality community healthcare services.

Main duties of the job
  • Lead and support medicines optimisation and pharmaceutical processes across Provide services to ensure they are safe, effective, patient-centred, and evidence-based.
  • Provide expert clinical pharmacy advice to multidisciplinary teams, supporting the safe management and optimisation of medicines for patients receiving community healthcare services.
  • Lead the pharmaceutical aspects of Sexual Health Services, including oversight of medicines governance, service data review, and statutory reporting requirements.
  • Act as the professional lead for Non-Medical Prescribing, supporting governance, safe prescribing practice, competency development, and service improvement.
  • Support the identification, management, and mitigation of medicines-related risks, contributing to organisational assurance and patient safety.
  • Drive quality improvement initiatives, audits, and service evaluations to enhance medicines use and patient outcomes.
  • Contribute to the development and redesign of patient pathways, ensuring medicines optimisation is embedded within service delivery models.
  • Work in partnership with clinical and operational teams to support innovation and service transformation across community healthcare services.
