Job summaryWe are looking for a motivated and enthusiastic Physiotherapist to join the Home First Service within Doncaster. As part of our integrated community care offer, we offer a community physiotherapy service alongside partners to support clients in their own home or residency. The current post would be based across Community Physiotherapy, Urgent Community Response and Enhanced Care Home Team, however, cover across the Home First Rehabilitation Service may be required at varying times.
The post holder will be required to develop skills that will support all areas within the Home First Rehabilitation pathways, which include:
- Community Physiotherapy
- Urgent Community Response
- Enhanced Care Home Team
- Community Rehabilitation Team
- Virtual Ward
- Intermediate Care Wards
The role is for Band 6 post, 37.5 hours per week, flexible working will be considered. The service is 7 days a week with bank shifts currently available as part of this role to support weekend service cover.
The Trust provides regular clinical supervision with a Personal Development Review. There is a range of Continuing Professional Development opportunities including in-service training, access to external courses, and a post graduate training support

Main duties of the job- To provide specialist assessment, diagnosis and treatment of patients with complex Physiotherapy problems as an autonomous practitioner within clinical speciality area
- To undertake line management responsibility for staff, within the service area to provide a responsive and effective service.
- To actively support the Team Leader in the operational management of the area and service developments on a local and/or service-wide level.
- To provide clinical education and training.
- To have responsibility for specific projects or initiatives contributing to the overall delivery of the service within a clinical speciality area.

Person SpecificationQualificationsEssential- Diploma/Degree in Physiotherapy
- Registration HCPC
ExperienceEssential- Demonstrable appropriate post graduate experience as a Band 5 physiotherapist in a broad range of areas including adequate relevant experience in the specific clinical specialty.
- Evidence of relevant CPD
- Evidence of community working
- Experience of working with older adults.
- Experience of supervising junior / support staff.
- An understanding of the principles of clinical governance
- Able to demonstrate an understanding of professional conduct and confidentiality
- Extensive experience of working as part of a physiotherapy and multidisciplinary team (MDT)
- Clinical education and development of physiotherapy staff and other members of the MDT.
Desirable- Experience of NHS working
- Experience in undertaking personal development appraisal.
- Leadership experience.
- Experience of research, audit, reflective practice and outcome measures.
- Experience of working with patients with a history of falls, and long term conditions and palliative conditions.
- Experience of networking within physiotherapy and in the wider Health & Social Care community
KnowledgeEssential- Understanding of current NHS issues and initiatives.
- Awareness of current relevant clinical guidelines e.g. Falls guidelines.
- Demonstrate clinical knowledge of a broad range of diagnoses and conditions in line with areas of the post
- Understanding of the issues pertaining to clinical risk management, and ability for formulate plans to reduce risk
- Demonstrable knowledge HCPC standard of practice and of professional registration requirements
SkillsEssential- Ability to communicate complex information clearly to a variety of professionals and to patients and their families, overcoming barriers to communication.

- Confident user of computers and IT databases. Proficient in Word, Excel and Power point.
- Demonstrate flexible approach to meet needs of patients and service delivery
- Able to recognise own limitations, when to seek supervision, and implementation of reflective practice
Desirable- Understanding of the Home First pathways.
- Completed student practice placement educator course
- Experience of project work at departmental level
- Ability to work in collaboration and positively influence or coach others.
