Surrey and Borders Partnership NHS Foundation Trust is looking for someone who is passionate about ensuring a high quality holistic approach to the care and support provided to clients in the community who have been identified as in need of community nurse support to enable them to exercise choice over their own lives.
Working Pattern: Monday to Friday from 9 am to 5 Pm (3 months)
Location: 18 Mole Business Park, Randalls Road, Leatherhead, Surrey, KT22 7AD
Main duties of the job- To provide high-quality, holistic, person-centred care to service users in the community.
- To lead the clinical management of a defined caseload, ensuring care is effectively planned, implemented, and evaluated.
- To coordinate and deliver evidence-based interventions, including CBT-informed therapeutic support.
- To work collaboratively within the multidisciplinary team (MDT) and with external agencies.
- To undertake comprehensive assessments, including risk assessments, and respond appropriately to changes in clinical presentation.
- To monitor and support both the physical and mental health needs of service users, including undertaking clinical observations.
- To promote recovery, independence, and choice, enabling service users to exercise control over their lives.
- To ensure safeguarding responsibilities are met and risks are managed in line with policies and procedures.
- To maintain accurate, timely, and legally compliant clinical records.
- To communicate effectively with service users, carers, families, and professionals.
- To contribute to service development, quality improvement, and audit processes.
- To support and contribute to staff development, supervision, and learning.
- To ensure compliance with professional standards, including the NMC Code, and Trust policies.

This role is working within i-access, a successful, countywide drug and alcohol treatment service for Surrey residents delivering tier 2, 3 and 4 substance misuse interventions across 3 main bases and 25 satellite locations. i-access is run collaboratively by SABP, PH / SCC and Via under a cooperative agreement which is one of partnership and transparency and contributes to Surrey Heartlands wider strategic ambitions.
Job responsibilitiesClinical Responsibilities- To lead the clinical management of care packages for a defined group of service users.
- To assess, plan, implement and evaluate care in line with evidence-based practice and Trust policies.
- To coordinate and deliver person-centred care, using advanced clinical decision-making where appropriate.
- To deliver therapeutic interventions, including individual and group work informed by CBT principles.
- To provide holistic care addressing physical, psychological, and social needs.
- To undertake and record physical observations (e.g. blood pressure, pulse, temperature, blood glucose, urinalysis) and respond appropriately to any concerns.
- To monitor changes in service users health and wellbeing and escalate concerns as required.
- To maintain professional boundaries while offering therapeutic support.
- To respect and promote service users diversity, dignity, values, and beliefs.
Communication- To maintain accurate, contemporaneous clinical records in accordance with Trust policy and professional standards.
- To communicate effectively with service users, carers, families, and professionals.

- To ensure valid consent is obtained prior to interventions.
- To ensure all communication complies with relevant legislation, policies, and procedures.

Person SpecificationExperienceEssential- Further training or experience to post graduate diploma level or 5 years equivalent experience
- 2 year's experience post qualification
- Aware of personal and professional boundaries
- Clinical supervision training
QualificationsEssential- RMN/RN (Mental Health)
- Successful completion of Mentorship course
