Job summary
The Clinical Nurse Specialist for the pain-related Complex Cancer Late Effects Rehabilitation Service provides expert assessment, management, education and support for people experiencing persistent pain, functional impairment and other long-term consequences of cancer treatment.

In this role you will work autonomously within the multidisciplinary team, managing a specialist caseload and coordinating personalised rehabilitation and supportive care pathways.

You will deliver specialist clinical care through face-to-face, virtual and telephone consultations, providing comprehensive assessment, triage, care planning and ongoing support.

The role focuses on optimising symptom management, improving function, promoting self-management and enhancing quality of life for people living with the consequences of cancer and its treatment.

In addition to clinical responsibilities, the post holder will contribute to service development, education, quality improvement, audit and research activities, supporting the continued growth and evaluation of Complex Cancer Late Effects Rehabilitation Service and promoting excellence in cancer rehabilitation and survivorship services.

Main duties of the job
  • Manage an autonomous specialist caseload within CCLERS.
  • Undertake specialist assessment, triage and care planning for people living with persistent pain, functional impairment and other long-term consequences of cancer treatment.
  • Coordinate complex patient pathways across primary, secondary and tertiary care services.
  • Work collaboratively within the multidisciplinary team to support the delivery of high-quality patient-centred care.
  • Act as a specialist resource for patients, carers and healthcare professionals regarding cancer rehabilitation and survivorship.
  • Support the development, evaluation and expansion of the CCLERS service, identifying opportunities to improve patient outcomes and service delivery.
  • Contribute to education and training programmes for healthcare professionals, patients and carers.
  • Promote awareness of the long-term consequences of cancer treatment and available rehabilitation pathways across the Trust and wider healthcare community.
  • Participate in audit, research, quality improvement and outcome evaluation activities to support service development and evidence-based practice.
  • Maintain accurate clinical records and contribute to collection of service activity and outcome data.

Person Specification
Qualifications
Essential
  • UK Registered Nurse with current NMC registration
  • Degree in Nursing or equivalent experience
  • Relevant post registration experience in cancer care, rehabilitation, pain/long term condition management
  • Teaching and presentation skills

Desirable
  • Masters or working towards Masters level study.
  • Independent Nurse Prescriber qualification
  • Formal qualification in Cancer Care, Survivorship or Rehabilitation.
  • Leadership qualification

Knowledge and Experience
Essential
  • Significant post registration clinical experience
  • Experience of working with people affected by cancer and its treatments and/or long term condition management
  • Evidence of excellent organisation, communication and presentation skills
  • Experience of autonomous clinical practice and caseload management

Desirable
  • Experience of working in cancer survivorship, cancer rehabilitation or pain management
  • Experience of service development projects
  • Experience of teaching, mentoring or clinical supervision
