RoleWe are seeking enthusiastic and patient-focused GPs who thrive in a dynamic and rewarding clinical environment. This opportunity is within an innovative @Home service, delivering hospital-level care to patients across Lambeth and Southwark.
Working alongside clinical teams from one of London's leading hospitals, you will play a key role in supporting admission avoidance and early supported discharge, helping patients receive safe, effective care in their own homes while reducing unnecessary hospital admissions and length of stay.
The post-holder will provide comprehensive GP care to patients within their homes, managing a varied caseload and addressing a wide range of health needs while maintaining the highest standards of patient care.
You will work as part of a highly collaborative multi-disciplinary team, including GPs, Advanced Clinical Practitioners (ACPs), specialist nursing teams, hospital consultants, therapists, social care professionals, and community services. Clinicians have direct access to the hospital MDTs and SELDOC's wider clinical network, ensuring patients benefit from coordinated and integrated care.
We recognize that many clinicians may be new to the Hospital @Home model. Successful applicants will therefore receive a comprehensive induction program, ongoing clinical support, and dedicated training to help them develop confidence and expertise within the service.
A unique benefit of this role is that a dedicated driver is provided for every shift, allowing clinicians to focus entirely on patient care while being transported between appointments.
Patients remain registered with their usual GP practice; however, the post-holder will work closely with the @Home nursing teams to provide medical support and clinical leadership, coordinating care across primary, community, social care, and secondary care services.
Base: @Home community bases (with occasional attendance at other locations as required)
Service availability: Sessional shifts within daytime operational windows of up to 12 hours.
Fees (outside IR35): £73.50 per hour (weekdays) £84 per hour (weekends)
