Job summary
We have an exciting opportunity for an Advanced Clinical Nurse Practitioner [ACP] to join our Memory Service multi disciplinary team. This is a primarily clinical role focused on the assessment, diagnosis and management of patients presenting with a cognitive impairment.

Main duties of the job
As the ACP will work autonomously to deliver expert clinical intervention and care planning, through your specialty knowledge and skills, providing an independent review of treatment plans and advising and supporting the B6 Assessors in the team. You will work closely with the team manager to provide clinical leadership, advice and training to the team and be responsible for ensuring the delivery of evidence based practice within the service, bringing the four pillars of advanced practice into working practice to help maintain and improve the quality of the service provision. at the MDT meetings.

Job responsibilities
Working across traditional roles within integrated care systems, the Advanced Clinical Practitioner (ACP) manages the complete clinical care for their patients utilising expert knowledge base and clinical competence to support multi-professional teams within Sheffield Health and Social Care, ensuring a high standard of clinical care, in accordance with the Nursing and Midwifery Council and Health and Safety Legislation.

As a clinical leader, the ACP has the freedom and authority to act autonomously in the assessment, diagnosis, treatment, including prescribing within an agreed scope, of patients with multi-dimensional problems and can work in or across all clinical settings, dependant on their area of expertise.

Range and Scope:
ACPs are educated at masters level in advanced practice and are assessed as competent in this level of practice. As a clinical leader they have the freedom and authority to act and accept the responsibility and accountability for their actions. This level of practice is characterised by high level autonomous decision making, including assessment, diagnosis, treatment including prescribing, of patients with complex multi-dimensional problems. Decisions are made using high level expert, knowledge, and skills.

ACPs have developed a highly specialist knowledge across a range of work practices and procedures underpinned by theoretical and practical knowledge and experience. The post-holder will be expected to work across traditional roles, providing complete clinical care for their patient as part of a multidisciplinary team.

Person Specification
Training and Qualifications
Essential

• NMC or HCPC registration & MSC in Advanced Clinical Practice
• Non-medical prescribing and ability to work within PGD's
• Minimum of 5 years post registration clinical experience

Desirable

• Recognised qualification in Clinical Supervision and/or Training and Assessment and/or Leadership

Knowledge and skills
Essential

• Demonstrate ability to assess and manage presentation to a high professional standard including formulation and diagnosis, collaborative care planning, monitor and review

Experience
Essential

• Demonstrate clinical leadership, leading service development and change
• Demonstrable experience of providing effective clinical supervision

Desirable

• Experience of working in a diverse range of care settings/services
