Job summaryAn exciting opportunity has arisen for a highly motivated Registered Nurse to join the South East District Nursing Team, in the provision of Planned and Admission Avoidance care to patients in the community.
We are looking for a caring, motivated friendly community nurse to join our busy, professional and dynamic team. You should possess commitment to interdisciplinary team working and innovative delivery of excellent standards of care. You will be able to demonstrate a real enthusiasm for community nursing, as well as a desire for on-going professional development.
Main duties of the job- Promotes a 'can do' attitude and determination to develop and sustain a dynamic and responsive administrative and operational service within a community setting which delivers best practice.
- Able to assess, plan, implement and evaluate interventions, liaising with colleagues and partners to ensure an integrated approach to the needs of the service user.
- To organise and deliver skilled clinical support work without direct supervision. This will include individual in a variety of settings.
- Gain valid informed consent and work within legal frameworks with patients.
- To liaise with senior clinicians with regard to patient care, highlighting any variations to expected outcomes from agreed treatment programmes.
- To provide basic verbal and written clinical reports and document patient interventions/findings in case notes in line with professional body and Trust standards.
- To instruct and educate patients, carers and families on a range of therapeutic activities.

Person SpecificationExperienceEssential- Previous community experience
- Experience of working in a multi disciplinary / multi agency
- Experience of successful partnership working arrangements with other services
Knowledge and SkillsEssential- Uses evidence-based practice
- Awareness of changing healthcare factors influencing change
- Well refined communication (verbal and written), interpersonal skills - ability to gain rapport at all levels
- Engaging and empowering approach that seeks to work with others and deliver outcomes in a successful and well informed
Training and QualificationsEssential- Registered Nurse on the NMC register
- Ability to demonstrate a level of English language and grammar commensurate with writing in service user case notes
- Commensurate with writing in service user case notes
