Description

Job summary
The GP role is sat within the medical team as part of a wider Multi-professional service at South Bristol Rehabilitation Units - based in South Bristol Community Hospital. You will primarily support the medical function(s) of the service ensuring that;
  • Patients are medically optimised to engage within their rehabilitation and recovery whilst an inpatient
  • New patient deterioration is flagged and managed appropriately to ensure that patients receive the right care in the right place, whilst promoting admission avoidance where appropriate
  • Non-acute medical issues continue to be proactively managed within a CGA and stroke framework to continue the patient's recovery, support managing long term conditions and reduce the risk of patient deterioration on discharge from the inpatient unit.

The units consist two 30 bedded wards, aimed at providing a safe environment where individuals who are 'medically stable' can receive specialist rehabilitation along with focused therapy and support from our multidisciplinary team, to support independence before they return home.

One ward is specifically a Stroke step down facility and the other accepts admissions from all other clinical backgrounds.

The stroke specialist unit has direct input from a Stroke consultant twice weekly, so the role is on both units is specialist frailty GP role given the wider frailty of the cohort of patients on both units.

Main duties of the job
  • Provide senior oversight for the medical input for the cohort of inpatient service users within the ward.
  • Use broad medical knowledge to manage a range of clinical presentations that will sit outside of the regular cohort of conditions that are routinely managed within the Advanced Practice scope of practice and conditions regularly seen in primary care - including and not limited to; skin disease, rheumatological conditions and management of heart failure.
  • Use a holistic approach to ensure factors within the Biopsychosocial model are considered
  • To consider whether there may be any reversible factors contributing to presenting conditions, including a thorough review of medication, in conjunction with pharmacy colleagues.
  • Medicines optimization and de-prescribing to reduce burden of medication for older people.
  • Initiating further investigations for abnormal blood tests, ordering radiology interventions including X-ray, USS and CT scans.
  • Referring, if clinically indicated via 2 week wait for suspected cancer and referring or liaising with other specialist services within secondary care and specialist services.
