Job summary
This is a fixed term post to support the ICSS team through times of high demand. ICSS is a supportive and friendly MDT providing assessment and rehabilitation for people in their homes and local community. Supervision would be provided by stroke specialist clinicians and you would have access to inservice training and advice from clinicians in the team. This is a great opportunity to develop your skills in stroke rehabilitation and occupational therapy

Main duties of the job

• To provide specialist assessment and rehabilitation for people with stroke in their homes and local community
• To work with the MDT to support people's transfer of care from hospital to home enabling flow from the hospital setting
• To work closely with other professions to provide holistic care to patients and their families
• To provide informal supervision for rehabilitation support workers and students
• To set up and monitor treatment plans for personal care, meal preparation and therapy programmes for support staff to follow
• Collaborate and negotiate with service users, carers and family in identifying appropriate and achievable goals as part of the overall care plan, which will maximise quality of life and independence.
• Facilitate appropriate referrals to other services or investigations based on your assessment to ensure holistic patient care
• To assess and arrange the provision of equipment and/or adaptations from appropriate agencies ensuring this is provided in good working order and fitted correctly; reporting any issues appropriately

Qualifications and Training
Essential

• Relevant occupational therapy qualification and HCPC registration
• Evidence of on-going CPD / Reflective Practice
• Working knowledge of OT outcome measures
• Knowledge of the principles of Clinical Governance and its application

Desirable

• Up to date knowledge of Evidence Based Practice, local and national standards

Skills, Knowledge and Experience
Essential

• Able to make clinically reasoned judgements when working with a variety of conditions
• Effective time management and caseload management
• Competent assessment and treatment skills including manual therapy, exercise prescription, directing a class
• Able to adapt communication for individuals with communication difficulties

Desirable

• Experience in the specialist area of stroke
• Experience of the application of health, safety and risk management policies
