Job summary
SPA is looking for a Community Specialist Practitioner who shares its vision to progress remote clinical decision making within the community and provide a highly effective Urgent Community Response.

SPA have been delivering a more effective urgent response, promoting, and championing a "Home is Best" ethos. We are working closely with system partners to deliver new ways of working such as Frailty-ACE and direct pathway from SWAST to Sirona. This role provides a key component to these services and will work onsite with Brisdoc at Osprey Court.

We are utilising our Community Specialist Practitioners to provide remote clinical decision making and using their skills to best support patients. We are looking for a partner to support us in continuing to change the way we work to meet new demands and within this role there will be opportunities to develop your leadership skills, share your knowledge and passion for community practice, whilst also having opportunity to help shape and develop the service and your role within.

Main duties of the job
  • Provide remote urgent assessment, treatment, and diagnosis of clinically deteriorating service users.
  • Triage and evaluate clinical information from examination and history taking and initiate appropriate treatment and/or referral.
  • Signpost appropriately to other services in the identified healthcare pathway, such as specialist services.
  • Provide a service that focuses on achieving and demonstrating overall improvements in health care for people who are complex and frail, with long term conditions who may present with urgent care needs.
  • Provide robust clinical leadership to ensure high quality harm free care.
  • To promote and champion use of digital technology in remote clinical assessment and decision making.
  • Work collaboratively with the other CSPs
  • Be responsible for the provision of relevant clinical training within the team which may include;
  • Supporting team performance in key identified areas including frailty and the assessment and management of long term conditions.
  • Support the development of advanced clinical practice within the team through the provision of education and clinical supervision.
  • Facilitate local and organisational audits, clinical investigations and RCAs relevant to clinical practice.
  • Work as an autonomous practitioner, actively using and promoting shared decision making and empowerment of service users, ensuring they have comprehensive, individualised care plans.

This post is not an Advanced Practice role that is mapped to the HEE Multi-professional Framework for Advancing Practice. These posts are currently under review and are likely to undergo a name change in the near future. This is because these roles do not require candidates to work across all 4 pillars of Advanced Practice to MSc level and are therefore banded at AFC Band 7. They do, however require a skillset working at or towards MSc level in the Advanced Practice Clinical Pillar. This role will not be eligible for training funding for an MSc in Advanced Practice however training may be supported on a modular basis depending on individual learning needs identified following Sirona CPD funding application processes.

Person Specification
Skills, Knowledge & Experience
Essential
  • Expert knowledge of complex case management
  • Developed skills and experience of managing service users with long term conditions and urgent care needs
  • Experience of using clinical examination and diagnostic reasoning skills
  • Experience in a relevant primary care, community or secondary care setting.
  • Ability to work with a high level of autonomy and shared decision making.
  • Ability to effectively communicate complex clinical information.
  • Experience of working across organisational and professional boundaries to improve care, including challenging service delivery and provision.
  • Experience of managing and leading people.

Desirable
  • Experience of remote assessment
  • Experience of clinical risk assessment

Qualification and Training
Essential
  • Successful completion of a relevant post-graduate L7 (or higher) course or appropriate equivalent modules.
  • Evidence of continuous personal and professional development and the commitment to undertake level 7 masters academic learning that supports the skillset required for this role.
  • Knowledge of local, national, strategic and operational policy developments and an awareness of their implications.
  • Willingness to undertake training required for the role.

Desirable
  • Post registration module at master's level.
  • Coaching / Teaching and Assessing qualification / experience.
  • Understanding of the primary care, secondary care and provider interface.
  • Knowledge of legal issues relating to the care of people such as Data Protection Act, Adult Protection procedures and the Mental Capacity Act
