Job summary
We are seeking a highly motivated, passionate Learning Disability Nurse (RNLD) to join the Intensive Support Team (IST) in Calderdale. We would welcome applications from newly qualified nurses, you would be supported in your new role under our preceptorship programme.

The IST is a small multidisciplinary team, working proactively and intensively with individuals with behaviours that challenge. We provide specialist assessments and interventions to support service users to remain in their home. We work closely with carers, support networks and wider agencies to develop their understanding of service users needs. Our work is informed by the principles of Positive Behaviour Support (PBS). There will be opportunity to undertake an accredited qualification in PBS.

The IST is part of the Community Learning Disability Health Team, a friendly and welcoming MDT. There is an emphasis on multidisciplinary working, with positive working relationships between professional groups. We have strong and effective links with our partner agencies including the Local Authority and ICB as well as local care providers.

Based in Hebden Bridge, a vibrant town on the edge of picturesque countryside we benefit from free parking in the town centre. We can offer flexible working hours and an agile working policy, with opportunities for home working as well as alternative bases to support effective cover of the wide geographical area of Calderdale. Part time working hours will also be considered.

Main duties of the job
The post holder will be responsible for a small caseload of clients of all levels of ability held jointly across the IST. This involves people presenting with behaviours that challenge, and responding to service users in crisis, working to prevent placement breakdown and hospital admission. The post holder will complete behavioural assessments and work with families, carers, staff teams and other professionals to implement positive behavioural support plans.

The post holder will be a member of a supportive multi-disciplinary team where there is an emphasis on working collaboratively with colleagues. There is also a requirement to work independently, making autonomous decisions.

A key aspect of the post is working closely with other agencies to bring about successful outcomes for our service users.

There will also be opportunities for continued professional development and also to contribute to the supervision of junior staff and students where appropriate.

Job responsibilities
JOB SUMMARY
JOB PURPOSE: The post holder will be responsible and accountable for the assessment, planning, implementation and evaluation of skilled interventions to address identified needs and risk for allocated clients on a defined caseload. Works within a multi-disciplinary team.

KEY RESULT AREAS:
  • To work as a member of an integrated multi-disciplinary team providing comprehensive assessment of clients and their carers, planning, implementing and evaluating skilled interventions to address identified needs.
  • To have knowledge of current issues and research relevant to the legislative framework and systems in which mental health care is delivered e.g. CPA, MHA (1983).
  • To contribute to service development within the clinical governance structure and policies and procedures of the Trust. Adhering to NHS guidance outlined by the relevant statutory registration bodies.

Professional
  • Assesses needs and identifies problems relevant to the care of people referred. Devises a plan of care in partnership with the person and carers. Implements the planned programme of care to ensure a high standard is achieved. Reviews the effectiveness of the care provided and where appropriate initiates any action.
  • To be competent in all aspects of care and risk management as defined within the framework of the care programme approach.
  • To work collaboratively with clients, carers and multiple other agencies to provide effective care to optimize and maintain clients mental health / learning disabilities and overall functioning, ensuring needs are met.
  • Management of enquiries and direct referrals of people in accordance with the \*\*\*\*\* Team Operational Policy and participates in the team duty rota. To participate in the team duty rota.
  • Maintain professional registration and demonstrate commitment to statutory codes of practice and continued professional development requirements.
  • To be responsible for the safe custody, transportation and administration of medicines including I.M. preparations.(if appropriate to qualification)

Education
  • Provide peer supervision and support to other team members and to take personal responsibility and supervise junior staff in the maintenance of written and electronic client and Trust records including inputting data related to own work activity. To identify any areas of concern to a senior member of staff.
  • To have an understanding of and participate in the clinical audit process.
  • To have an awareness of evidence based practice relevant to the specialist area and participate in the education of individuals and target group.
  • To participate in the Trusts values based appraisal system, understanding appropriate training and participating in personal development opportunities.

Qualifications
Essential
  • Registered Learning Disability Nurse
  • Or Registered Mental Health Nurse with significant Learning Disability experience.

Desirable
  • Experience of mentoring/supporting learners or ability to demonstrate the skills required for effective mentoring

Training
Essential
  • Assessment and care planning skills and risk assessment and risk management.

Desirable
  • Relevant Post basic qualification and training

Experience
Essential
  • Can demonstrate skills in assessing and managing people experiencing mental health problems / learning disabilities (high levels of emotional distress and/or challenging behaviour)
  • Can demonstrate skills in assessing and managing risk in a variety of settings
  • Proven ability in assessing the needs and strengths of people and their carers
  • Ability to deliver a range of therapeutic techniques which can be incorporated into one to one and / or group interventions
  • Working within an integrated multi-disciplinary environment and co-working with other teams within team meetings, case conferences and professional meetings

Desirable
  • Ability to support colleagues and supervise students/trainees
  • Experience of working holistically with adults with an emphasis on coexisting mental and physical health needs that impacts on wellbeing

  • Knowledge of working collaboratively with people and their carers to ensure their involvement in the assessment and care planning process
  • Shows awareness of cultural diversity and is able to incorporate this into care planning and the delivery of care
