An exciting opportunity has arisen for a part time Band 6 Occupational Therapist within the Equipment, Adaptation & Sensory Impairment team based at Hoyland

We are a small & friendly team of Occupational Therapists, Equipment & Adaptation Officers, Administration staff & Sensory workers.

We assess people of all ages who have a wide range of medical conditions and physical and sensory disabilities and carry out outcome focused needs led assessments.

We use person centred methods that respect individual values, cultures and diversity to identify needs and devise individual interventions to meet need this could be via provision of equipment or minor or major adaptations and/or offer information and advice where statutory provision is not appropriate.

We currently have a blended way of working between base & home which gives flexibility.

Main duties of the job
You will work autonomously in the community and manage your own clinical caseload.

You will cover the whole of Barnsley & you will assess service users of all ages in their own homes.

You will work closely with our partners in health & housing when recommending equipment, minor and major adaptations and rehousing.

You will participate in the OT duty rota prioritising in-coming referrals, assessing risk to determine action and allocating referrals for immediate action where necessary.

You will carry out functional activity of daily living assessments, moving and handling assessments and vacant property assessments.

You will receive regular supervision & participate in training & development & team meetings. You will also support your colleagues with supervising junior members of staff and students and carrying out staff development reviews.

Job responsibilities
JOB SUMMARY

• To see people of all ages who have a wide range of medical conditions and physical and sensory disabilities; to carry out outcome focused needs led assessments
• Use person centred methods that respect individual values, cultures and diversity to identify needs and devise individual interventions to meet need and/or offer information and advice where statutory provision is not appropriate
• To be responsible for supervising the work and development of other members of the team including, junior occupational therapists, equipment and adaptation officers and students
• To participate in specific projects to support the development of the service

KEY RESULT AREAS:

• Work as a lone practitioner in a community setting with service users to analyse physiological, psychological and environmental factors to develop individual goal orientated action plans.
• Take into account the views of the person and their carer(s) to negotiate and agree appropriate provision of services, equipment, minor and major property adaptations or re-housing that is necessary to meet current and long-term needs whilst considering the legal framework, feasibility, practicality and resource implications of the proposed plan.
• Carry out holistic risk assessments and take action to reduce and manage risk to people in their homes advising them and their carer(s) on various aspects of safe independence or care. This can involve assessing, advising, demonstrating and giving guidance on moving and handling techniques and supporting a rehabilitation programme for both formal and informal carers.
• Work autonomously to manage own clinical caseload. Recognise specific areas of concern and the need to re-prioritise cases according to circumstances. Refer people to other services and agencies as required.
• Develop and maintain a current working knowledge of the safe use of a wide range of equipment for both physical and sensory impairment. Transport, assemble and fit equipment at peoples homes to assess their appropriateness and safe use within the home environment. Train service users and their carer(s), both informal and formal, in the correct and safe methods of using equipment provided.
• Apply knowledge of the legislative framework and policy related to people with physical and sensory impairments and support the specialist resource within the context of individual and specific needs, and also represent the service within this context.
• Provide regular clinical and case supervision to junior staff and carry out staff development reviews.
• Participate in the senior OT duty rota prioritising in-coming referrals, assessing risk to determine action, allocating referrals for immediate action where necessary
• Monitor the case load management of junior colleagues
• Agree rehabilitation goals and identify the duration and type of training programme or other intervention needed to maximize the service users' independence
• Supervise Occupational Therapy students and participate in their training
• Complete written reports of assessments and recommendations, to inform relevant others of individuals circumstances and specific needs.
• Attend case conferences with multi-disciplinary agencies when required and take shared responsibility for the agreed outcome.
• Keep clear, concise, accurate records according to professional standards and local policy. Regularly record statistical information to provide information needed for Performance Indicators.
• Participate in clinical governance through supervision, and research and development to ensure the service is effective and providing best value.
• Give advice and guidance to other staff in dealing with enquiries from colleagues and service users as they arise.
• Be responsible for reviewing and reflecting on own practice and performance and for own continuing professional development. In accordance with Trust policy to identify and undertake appropriate training and development.
• Participate in the training of staff within the team and of other agencies as and when required.
• Undertake other duties as appropriate to the post and grade as required for service delivery
• Assist in maintaining own and others health, safety and security
• Undertake training and development opportunities as required by the line manager.

COMMUNICATION WITH OTHERS:
With service users, their carers and relatives mainly face to face but also phone, written and other relevant communication methods specific to the type of service provision.

Discuss implications and impact of medical condition/disability, frequently deteriorating or terminal. Explore and negotiate reasonable action often in life changing circumstances and complicated family dynamics.

Utilise various methods including detailed and complex written reports to communicate frequently with:

• Colleagues across health services and social services
• Regular contact with voluntary organisations
• Independent sector; suppliers and contractors
• Housing officers, surveyors, architects and buildings officers of Berneslai Homes, the Disabled Facilities Grants Team, BMBC Direct Works and Housing Associations.

Use IT and electronic communication where appropriate and support electronic record keeping

Person Specification
Qualifications
Essential

• Degree or equivalent in Occupational Therapy.
• Current registration with HCPC.

Training
Essential

• Evidence of Continuing Professional Development and training in relevant area.

Desirable

• Accredited practice educator (or willingness to undertake).
• Approved moving and handling training.

Special Knowledge/Skills
Essential

• Ability to demonstrate specialised clinical reasoning and treatment skills.

Experience

• Successful completion of a post registration junior rotation including:
• relevant post graduate training;
• work in various fields covering a wide range of medical conditions;
• planning and facilitating safe discharge from hospital or implementing interventions to prevent admission;
• supervising junior colleagues or students;
• moving and handling adults;
• assessment and recommendation of specialist equipment and major adaptations.

Desirable

• working with children in a community setting.
