Southview Medical Practice is a well-established, patient-focused GP surgery serving the local community in Woking. We are looking to recruit an enthusiastic, caring, and motivated Practice Nurse to join our experienced clinical team.

Main duties of the job
Main Duties and Responsibilities

The successful candidate will be responsible for providing a wide range of nursing services, including:

• Chronic disease management (including asthma, COPD, diabetes, hypertension and cardiovascular disease)
• Cervical cytology (smears)
• Childhood and adult immunisations and vaccinations
• Wound care and dressings
• Travel health advice and vaccinations
• Health promotion and lifestyle advice
• NHS Health Checks
• Venepuncture
• ECGs and other clinical procedures
• Supporting infection prevention and control processes
• Maintaining accurate clinical records using the practice clinical system

Job responsibilities
Southview Medical Practice is seeking an enthusiastic, motivated, and patient-focused Practice Nurse to join our friendly and supportive multidisciplinary team. The successful candidate will provide high-quality nursing care to our registered patient population, promoting health, preventing disease, and supporting the management of long-term conditions within primary care.

The post holder will work collaboratively with GPs, healthcare assistants, pharmacists, and other healthcare professionals to deliver safe, effective, and compassionate care in accordance with local and national guidelines.

Main Duties of the Job

The Practice Nurse will be responsible for delivering a range of evidence-based nursing services, including but not limited to:

• Long-term condition management, including asthma, COPD, diabetes, hypertension, and cardiovascular disease.
• Cervical cytology screening.
• Childhood, adult, seasonal and travel immunisations and vaccinations.
• Wound care, dressings, and removal of sutures/clips where appropriate.
• Venepuncture and specimen collection.
• NHS Health Checks and health promotion activities.
• ECG recording and other clinical procedures.
• Lifestyle advice and support relating to smoking cessation, weight management, physical activity, and general wellbeing.
• Supporting patients with preventive healthcare and screening programmes.
• Maintaining accurate, contemporaneous patient records using the practice clinical system.
• Ensuring clinical areas and equipment are maintained in accordance with infection prevention and control standards.
• Participating in clinical audits, quality improvement activities, and practice development initiatives.
• Working within professional competency and seeking support where appropriate.

Clinical Responsibilities

• Deliver evidence-based nursing care in line with NMC standards and practice policies.
• Assess, plan, implement, and evaluate patient care.
• Recognise and respond appropriately to safeguarding concerns.
• Support the management of patients with chronic diseases through monitoring, education, and treatment reviews.
• Identify and report significant events, incidents, or risks in accordance with practice procedures.
• Contribute to the achievement of Quality and Outcomes Framework (QOF) targets and enhanced services where appropriate.

Communication and Team Working
The post holder will:

• Develop and maintain positive working relationships with colleagues and external healthcare professionals.
• Communicate effectively and sensitively with patients, carers, and stakeholders.
• Work collaboratively within the multidisciplinary team to ensure coordinated patient care.
• Participate in team meetings and training sessions.

Professional Development
The post holder will:

• Maintain NMC registration and professional revalidation requirements.
• Participate in annual appraisal and personal development planning.
• Undertake mandatory and role-related training.
• Keep up to date with current clinical practice, legislation, and national guidance relevant to primary care nursing.

Person Specification
Experience
Essential

• Previous Practice Nurse experience.
• Experience using EMIS Web.
• Experience of QOF and Enhanced Services.

Desirable

• Experience of providing nursing care within primary care, community, or similar healthcare settings.
• Knowledge of chronic disease management.
• Experience of immunisations and vaccinations.
• Knowledge of infection prevention and control requirements.

Qualifications
Essential

• Registered Nurse with current NMC registration.

Desirable

• Cervical Cytology qualification.
• Independent Prescriber qualification.
• Respiratory, Diabetes, or Long-Term Condition Management qualifications.
