Job summary
We are looking for an advanced nurse practitioner to provide maternity cover on a 9 month contract. This is a part-time position (25 hours per week).

The successful applicant will be an experienced ANP who enjoys working in a busy practice and being part of a large multi-disciplinary, very supportive team.

Main duties of the job
The successful applicant will provide expert, person centred care for patients presenting with undifferentiated, acute and long term needs. Use advanced clinical reasoning to assess, diagnose, plan, treat, refer and arrange follow up. Work collaboratively with GPs and the wider team, escalating when a presentation exceeds personal competence or the agreed service pathway.

We are a large practice (16000 patients) and a large team of 4 GP partners, 4 salaried GPs, 3 ANPs, 5 APPs, pharmacists, physiotherapist, nurses, nursing associates, health care assistants all supported by an excellent admin team.

Job responsibilities
Main responsibilities

Advanced clinical practice

• Undertake comprehensive histories, examination, risk assessment and clinical reasoning for patients within the agreed age range and service scope; recognise red flags, deterioration and time critical illness.
• Formulate and explain differential diagnoses and management plans; request and interpret investigations where authorised and competent; arrange safety netting, review, referral or urgent escalation.
• Prescribe, supply or administer medicines only within current legal authority, NMC annotation where applicable, local policy and personal competence; undertake medication review and antimicrobial stewardship.
• Manage complex long term conditions, multimorbidity, frailty and prevention needs with shared decision making, continuity and coordinated care.
• Provide consultations in person, by telephone or digitally as appropriate, maintaining accurate contemporaneous clinical records and appropriate coding.
• Assess mental capacity and obtain valid consent; identify safeguarding concerns and act under local pathways, including timely documentation and referral.
• Recognise health inequalities and adapt communication and care for disability, language, culture and other individual needs.

Person Specification
Experience
Essential

• Must be able to treat patients from birth to death.
• Experience of women's health.
• Experience of treating mild/moderate MH issues.

Desirable

• Experience using System One clinical system.
• Able to perform triage/duty clinician.

Qualifications
Essential

• Current NMC registration without restrictions compatible with the role.
• Independent prescribing qualification.
• Advanced clinical skills.
