Job Summary
ANP/PARAMEDIC PRACTITIONER 16 - 20HRS Per Week. The Candidate will work as an autonomous practitioner within the clinical team, providing high-quality, patient-centred care. Must be an experienced and compassionate nurse with advanced clinical skills and the ability to demonstrate critical thinking while working within professional boundaries. They will make safe and effective clinical decisions, delivering expert assessment, treatment and evaluation for patients presenting with minor illness and ongoing medical needs. Demonstrate clinical judgement and evidence-based practice. Assess, triage, diagnose and treat patients through face-to-face consultations, undertaking comprehensive examinations and developing appropriate care plans. Request and interpret diagnostic tests in line with agreed protocols and refer patients to secondary care when required. Prescribe and review medications as an independent prescriber in accordance with national guidelines and practice protocols. Maintain patient confidentiality, dignity and safe clinical practice within professional competencies. Deliver compassionate, holistic care that promotes health, self-care and independence. Maintain accurate patient records, manage urgent consultations and contribute to practice targets and quality standards. Participate in staff training, professional development, audits and research activities while ensuring compliance with data protection, health and safety, infection control and equality policies.

Main Duties of the Job
  • The post holder will deliver high-quality, patient-centred care while working collaboratively with multidisciplinary teams to improve health outcomes and ensure safe, effective service delivery.
  • They will utilise advanced clinical knowledge and decision-making skills to assess patients presenting with acute, chronic, and complex conditions.
  • They will independently manage episodes of care, prescribe medications where qualified, order and interpret diagnostic investigations, and make appropriate referrals to specialist services when required.
  • Undertake comprehensive clinical assessments, including detailed history taking, physical examination, and risk assessment.
  • Diagnose and manage a wide range of acute, chronic, and complex health conditions.
  • Formulate differential diagnoses and develop evidence-based treatment plans.
  • Independently prescribe and review medications within professional competence and prescribing authority.
  • Provide timely interventions, treatment, and follow-up care to achieve positive patient outcomes.
  • Recognise deteriorating patients and initiate appropriate escalation and emergency responses.
  • Maintain accurate and contemporaneous clinical records in line with professional and organisational standards.
  • Deliver compassionate, person-centred care that respects patient dignity, preferences, and cultural needs.

About Us
St James Practice is a General Practice in Beckenham, Bromley providing NHS primary care services to over 7,000 patients. We have a well-established team of 4 GP Partners, 1 Salaried GP, 1 GP Retainer and 2 Practice Nurses. We are a training practice with a GP Registrar and occasional Medical Students. We are involved in new projects to enhance the delivery of primary care. We work in conjunction with our PCN onsite Clinical pharmacists, Dietician, Physiotherapist, Mental Health Practitioner and Health and Wellbeing Coach to expand the care we provide to patients. Job Responsibilities
  • Assess, diagnose, plan, implement and evaluate treatment or interventions and care for patients presenting with an undifferentiated diagnosis
  • Clinically examine and assess patient needs from a physiological and psychological perspective, and plan clinical care accordingly
  • Assess, diagnosis, plan, implement and evaluate interventions or treatments for patients with complex needs, proactively identify, diagnose and manage treatment plans for patients at risk of developing a long-term condition as appropriate
  • Diagnose and manage both acute and chronic conditions, integrating both drug- and non-drug-based treatment methods into a management plan
  • Prescribe and review medication for therapeutic effectiveness, appropriate to patient needs and in accordance with evidence-based practice, national and practice protocols, and within scope of practice
  • Work with patients to support compliance with and adherence to prescribed treatments. Provide information and advice on prescribed or over-the-counter medication, medication regimes, side-effects and interactions
  • Prioritise health problems and intervene appropriately to assist the patient in complex, urgent or emergency situations, including initiation of effective emergency care
  • Support patients to adopt health promotion strategies that promote healthy lifestyles and apply principles of self-care
  • Support and manage health needs of women presenting with issues relating to family planning, cervical cytology and sexual health
  • Assess, identify and refer patients presenting with mental health needs in accordance with the NSF for Mental Health Implementation
  • Participate in vaccination and immunisation programmes for both adults and children
  • Advise, support and administer vaccinations for patients travelling abroad
  • Promote and deliver evidence-based care for patients presenting with aural conditions

Person Specification
Qualifications
Essential
  • Registered Nurse/Paramedic with current registration with the relevant professional regulatory body.
  • Masters degree in advanced clinical Practice or equivalent advanced practice qualification.
  • Independent/Supplementary Prescribing qualification.
  • Evidence of ongoing professional development.

Essential Experience
  • Significant post-registration clinical experience.
  • Experience working at an advanced practice level.
  • Experience of autonomous clinical decision-making and patient management.
  • Experience of prescribing and managing complex patient presentations.
  • Experience working within multidisciplinary teams.

Essential Knowledge and Skills
  • Advanced assessment, diagnostic, and clinical examination skills.
  • Excellent communication and interpersonal abilities.
  • Strong leadership and mentoring skills.
  • Ability to prioritise workload and manage competing demands.
  • Sound understanding of clinical governance, safeguarding, and risk management.
  • Knowledge of current healthcare legislation, policies, and evidence-based practice.

Desirable
  • Eligible to work in General Practice in the NHS
  • Experience in primary care, urgent care, community services, or specialist clinical settings.
  • Non-medical prescribing experience in a relevant clinical area.
  • Experience leading service improvement or quality initiatives.
