Job SummaryThis is an exciting opportunity for an experienced Practice Nurse Manager or a Practice Nurse who currently works within General Practice, and is looking to develop their career in a busy practice in a dream location.
We would like to welcome an enthusiastic, committed and forward-thinking Practice Nurse Manager into our friendly team, to provide leadership, support to the nursing team whilst at the same time helping develop the nursing services offered by the Practice.
You will also work closely with the Clinical Team, Practice Manager and the wider Practice Team to help to meet the Patients needs.
Main duties of the jobAs a Practice Nurse Manager, you will work as part of the nursing and multidisciplinary team, supporting the delivery of chronic disease management, preventative healthcare, treatment room services and health promotion.
Key Clinical ResponsibilitiesYour duties will include, but are not limited to.
- Providing high-quality, evidence-based nursing care to patients.
- Providing Leadership and Support to the Nursing Team.
- Supporting the practice in achieving clinical quality indicators and targets.
- Providing care and monitoring for patients with long-term conditions, including diabetes and respiratory disease.
- Undertaking wound assessment, treatment and ongoing management in line with local and national guidance.
- Carrying out venepuncture and appropriate clinical observations.
- Administering intramuscular and subcutaneous injections, including vaccinations and other prescribed therapies.
- Providing adult and childhood immunisation services in accordance with appropriate PGDs/PSDs and practice procedures.
- Supporting travel health and vaccination services following appropriate training.
- Providing cervical screening.
- Undertaking NHS health checks and learning disability health checks for eligible patients.
- Performing INR checks for patients receiving warfarin, following appropriate training.
- Maintaining effective infection prevention and control procedures.
- Recognising and responding appropriately to safeguarding concerns.

Job responsibilitiesJob Summary
The post holder is responsible for ensuring the delivery of safe and effective nursing care to the whole practice population. As the team leader for the general practice nursing team, the post holder is accountable for nursing service delivery. They will lead and manage all the nursing resources, working closely with the GPs, Practice Manager and Development Manager to deliver the practice priorities. Clinically, the focus of the role is the delivery of evidence-based practice for patients with acute and chronic conditions and management and preventative nursing interventions to all patients.
You will work collaboratively with the whole general practice team to help meet the needs of patients, supporting the delivery and procedures and providing leadership and direction for the nursing team.
Scope and Purpose of the Role:
Working in accordance with the NMC Code of Conduct, the post-holder will lead and develop the practice nursing team in the provision of assessment, treatment, screening, health education services and advice to patients.
The post-holder will develop the nursing teamwork with reference to practice-agreed protocols and guidelines to provide nursing treatments to patients both independently and in participation with nurse colleagues and general practitioners.
The post-holder will have the required level of training and competence and will work within patient group directions where these are available
To help develop the nursing services offered by the practice.
To provide clinical leadership within the nursing team
To ensure the highest standards of care are provided for patients.
To manage and develop a clinical acute and chronic case load, dealing with presenting patients needs in a primary care setting.
Prioritise health problems and intervene appropriately to assist the patient in complex, urgent or emergency situations, including initiation of effective emergency care.
ManagerialResponsibility for the day to day running of the nursing team including liaising with the management team on rotas, holiday management, sick leave and any unscheduled events that can affect service delivery.
Responsible for support and pastoral care to the nursing team including HCAs.
Conduct and manage nurse appraisals with the Practice Manager on an annual basis ensuring learning needs are explored and planned for the year ahead. Also ensuring nurse revalidation and correct registration with the Royal College of Nursing.
Ensuring the nursing team are up to date, attending appropriate courses including mandatory training e.g. include but not limited to Safeguarding, Information Governance, Immunisations, Smears, once authorised by Practice Manager.
QOF liaising with the clinical leads and admin and the management team and ensuring the running of specialised chronic review clinics.
Leading and take part in Flu and Covid and annual immunisation campaigns. Working in tandem with the management team to deliver the service including providing a home visiting service to the housebound patients.
Ensuring PGDs and PSDs are up to date for Nurses and HCAs.
Leadership of vaccines, ordering, delivery and maintaining the cold chain.
Maintaining and overseeing the vaccine delivery.
Ensuring good working order of the resuscitation and anaphylaxis and any other equipment.
Oversight of all Nursing roles including those delegated to other nurses, retaining overall responsibility as nurse manager and identify the workforce that is required to provide services to meet the needs of the patient population and negotiate with the practice management team for this workforce.
Monitor and advise on the effective use of physical and financial resources by the practice nursing team and working with a budget to manage equipment ordering.
Assess, plan, develop, implement and evaluate programmes which promote health and well-being, and prevent adverse effects on health and well-being.
Help the practice develop and deliver specialist primary nurse led services including chronic disease management. Initiate the role of diagnosis for patients suspected to have a chronic disease e.g. diabetes, COPD, asthma and CHD, referring to other clinicians as appropriate.
Manage programmes of care for patients with chronic diseases by planning, providing, and evaluating care under agreed guidelines and patient group directions
Undertakes agreed procedures eg - blood pressure, pulse rate and rhythm, temperature, height and weight body mass index, venepuncture, ear syringing, ECG, peak flow readings, cytology, travel and childhood vaccinations, interpretation of blood results via the pathology link, and any other nursing procedure the practice deems necessary.
Be competent in anaphylaxis and resuscitation techniques
Prioritise health problems and intervene appropriately to assist the patient in complex, urgent or emergency situations, including initiating effective emergency care.
Review medication for therapeutic effectiveness, appropriate to patients needs and in accordance with evidence-based practice and national and practice protocols
Assess and care for patients attending with wounds
Implement and participate in immunisation programmes for both adults and children
Assist senior practitioners in providing minor surgery and women's health when required.
Make appropriate referrals to other members of the primary, community and secondary care teams
CommunicationUtilise and demonstrate sensitive communication styles, to ensure that patients are fully informed and consent to treatment
Communicate effectively with patients and carers, recognising the need for alternative methods of communication to overcome different levels of understanding, cultural background, and language differences
Maintain effective communication with individuals and groups within the practice environment and with external stakeholders
Act as an advocate when representing the patients and colleagues viewpoints to others
Recognise and work within own competence and professional code of conduct as regulated by the Nursing and Midwifery Council.
Produce accurate and complete records of patient consultation consistent with legislation, policies and procedures.
Will prioritise, organise and manage own workload in a manner that maintains and promotes quality working autonomously
Deliver care according the Stennack Surgery policies, and the National Institute for Clinical Excellence guidelines.
Assess effectiveness of care delivery through self and peer review, benchmarking and formal appraisal
Develop a suite of audits that can be utilise as a means of evaluating the quality of the work of self and team, implementing improvements where required
In partnership with other clinical teams, collaborate on improving the quality of healthcare, responding to local and national policies and protocols and initiatives as appropriate
Evaluate the patients response to health care provision and the effectiveness of care
Support and participate in shared learning across the practice and the wider organisation
Identify learning needs from patient complaints, clinical incidents and near misses utilising a structured framework
Participate in the performance monitoring of the team, providing feedback as appropriate to the Practice Manager.
Understand and apply legal policy the supports the identification of vulnerable and abused children and adults, being aware of the statutory health procedure and local guidance
Work within policies relating to domestic violence, vulnerable persons, substance and alcohol misuse; and refer as appropriate
Attend and contribute to relevant internal and external meetings as required including practice clinical meetings, nurse team meetings and any other meetings that may be required
Take responsibility for own and nursing team development, learning and performance, and arranging courses once authorised by Practice Manager. Including participating in clinical supervision and acting as a positive role model.
Contribute and participate in the development of local guidelines, protocols and standards
Keep up to date with new developments locally and nationally, identifying those that will enhance the teams or the organisations work
Work as an effective and responsible team member, supporting others and exploring the mechanisms to develop new ways of working
Manage and assess risk within the areas of responsibility, ensuring adequate measures are taken to protect patients and staff
