Job summary
Based at Berkeley House in Godalming , the Waverley Neighbourhood Mental Health FRAILTY, LATER LIFE & DEMENTIA service provides comprehensive assessment, diagnosis, treatment, and ongoing support for individuals experiencing functional mental health conditions, frailty, cognitive decline, and/or dementia.

We are seeking enthusiastic and motivated professionals who hold registration with a recognised professional body (NMC, HCPC, or SWE) to join our supportive, dynamic, and multidisciplinary team.

This role is ideally suited to an experienced clinician with advanced clinical expertise, strong leadership capabilities, and a passion for delivering high-quality, person-centred care. The successful candidate will be able to inspire, support, and motivate colleagues while driving excellence in service delivery and outcomes for those accessing our services.

Main duties of the job

• To support the service manager in the effective operation of the Frailty, Later Life and Dementia service line.
• To ensure the service operates in a safe, effective & efficient way as part of the whole Runnymede community neighbourhood team.
• To manage complex cases, providing leadership and supervision of staff and ensuring high quality evidence-based patient care.
• Being part of the community neighbourhood clinical leadership team -working collaboratively to ensure people's needs are met and a reduced need for repeat assessments.
• Assess patients/clients; plans, implements care; provides highly specialist advice; maintains associated records.
• May liaise with other agencies in planning programmes of care and/or health and education programmes

• To be responsible for the development and maintenance of effective clinical pathways within the service line and wider community INT team.

• Significant experience working at band 6 or higher.
• Professional knowledge acquired through degree supplemented by post registration/post graduate specialist training, experience and/or short courses, CPD or equivalent, including Practice Supervisor and Practice Assessor (PSPA) / mentorship qualification.
