Triage Access CoordinatorJob summaryWe are looking for an experienced Mental Health Practitioner from a background in Mental Health Nursing, Occupational Therapy, or Social Work to join our Brighton and Hove Neighbourhood Mental Health Team. As an experienced and Senior Mental Health Practitioner, you will have experience and knowledge of secondary care mental health services in order to support and oversee referrals to these services and identify appropriate pathways for service users in Brighton and Hove.
Main duties of the jobThe post holder will work alongside colleagues and take a lead in the daily Multi-Disciplinary Team (MDT) triage meetings and the Multi-Agency Triage Hub (MATH) to identify pathways and access for referrals to mental health services.You will have excellent communication and organisational skills and will need to develop positive working relationships with our primary care and third sector colleagues in order to streamline pathways and ensure that service users are accessing the most appropriate support and/or treatment for their identified needs in line with the ethos of Neighbourhood Mental Health Working.
You will demonstrate experience in identifying risk factors and be able to prioritise referrals accordingly. The role involves frequent communication with service users through principals of Active Referral Management, and communicating with referrers from external agencies and internal teams, and with other agencies as part of our multi-agency triage hub. You will also have a role in supporting more junior staff in the team and providing clinical leadership.
