Job summaryRotherfield and The Brook Surgeries are seeking an experienced and enthusiastic Advanced Clinical Practitioner (ACP) or Advanced Nurse Practitioner (ANP) to join our friendly and supportive team on a flexible bank basis, with the potential for a permanent position.
This role is ideal for an autonomous, experienced primary care clinician who enjoys managing undifferentiated presentations and providing high-quality patient-centred care. You will undertake telephone and face-to-face consultations, assess, diagnose and treat a wide range of acute and chronic conditions, prescribe independently, and work collaboratively within our multidisciplinary team.
Our practices use Total Clinical Triage to manage all incoming patient requests, and you will play a key role within this model. Full training and support will be provided to ensure you feel confident working within our systems and processes.
We are looking for clinicians with significant experience in primary care who are confident working independently while recognising the value of teamwork, clinical governance and continuous learning.

Main duties of the jobThe successful applicant will work as an autonomous Advanced Clinical/Nurse Practitioner within our multidisciplinary team, delivering safe, effective and patient-centred care across a broad range of clinical presentations.
You will undertake telephone and face-to-face consultations, assess, diagnose and manage patients presenting with undifferentiated and acute illness, provide appropriate treatment, prescribe independently, request and interpret investigations, and refer patients to other services where appropriate.
You will be expected to work within your scope of practice while demonstrating excellent clinical judgement and recognising when advice or escalation is required.
Our practices operate a Total Clinical Triage model, and you will contribute fully to this service following an appropriate induction.
In addition to direct patient care, you will maintain accurate clinical records using SystmOne, contribute to clinical governance and quality improvement activities, and work collaboratively with GPs, Nurses, HCAs, Pharmacists and the wider practice team to deliver high-quality care.
This is a flexible bank role providing cover for annual leave, sickness absence and periods of increased demand, with the opportunity for more regular sessions and the potential for a permanent position.

The successful applicant will be an experienced primary care clinician who is confident managing undifferentiated presentations, making complex clinical decisions and working independently within their scope of practice. The role involves delivering both telephone and face-to-face consultations within our Total Clinical Triage model, supporting same-day demand as well as planned care.
This post is intended for clinicians with established experience in UK General Practice. It is not suitable for newly qualified ACPs or clinicians requiring a preceptorship or a high level of supervision.

Main Duties and ResponsibilitiesClinical Responsibilities- Provide autonomous assessment, diagnosis and management of patients presenting with undifferentiated, acute and chronic conditions.
- Undertake telephone, video and face-to-face consultations as clinically appropriate.
- Independently prescribe within professional competence and current legislation.
- Request, interpret and act upon investigations.
- Develop evidence-based management plans.
- Refer appropriately to secondary care, community services and the wider multidisciplinary team.
- Recognise deteriorating patients and initiate appropriate escalation.
- Deliver health promotion, disease prevention and self-management advice.
- Undertake home visits where clinically appropriate.
Clinical Governance- Maintain accurate and contemporaneous records using SystmOne.
- Work in accordance with NICE guidance, national policy and local clinical protocols.
- Participate in audit, quality improvement and clinical governance activities.
- Report incidents and contribute to significant event reviews where appropriate.
- Maintain compliance with safeguarding, infection prevention, information governance and health and safety policies.
- Maintain professional registration, indemnity and continuing professional development.
Team Working- Work collaboratively with GPs, ACPs, Nurses, HCAs, Pharmacists and the wider practice team.
- Participate fully in the practice's Total Clinical Triage model.
- Communicate effectively to ensure safe continuity of patient care.
- Contribute positively to a supportive, respectful and learning-focused team culture.

- Sound knowledge of current evidence-based practice and NICE guidance.

QualificationsEssential- Current registration with the NMC, HCPC or other relevant UK professional regulator.
- MSc Advanced Clinical Practice (or equivalent advanced practice qualification).
- Independent Prescriber qualification.
- Evidence of ongoing Continuing Professional Development (CPD).
Desirable- Advanced Practice Digital Badge.
- RCN Credentialing (or equivalent advanced practice recognition).
- Additional postgraduate qualifications relevant to primary care.
