Hospital at Home Senior Nurse - Band 6 21 Hours - Permanent

This is an exciting opportunity for a skilled nurse to join the Hospital at Home Service working across the wider Division of Integrated Care, Adults, Children and Young People

Main duties of the job
As a member of the Hospital at Home service, you will be expected to work as a senior nurse with a broad range of competencies who is able to adapt to a wide variety of clinical circumstances. To work as part of a multidisciplinary team to complete clinical assessments, treatments and management of patients within their own homes as part of the Hospital at Home and community- based admission avoidance pathways.

The post holder will work autonomously within their sphere of competence, undertaking comprehensive assessments, formulating management plans, initiating treatment and coordinating multidisciplinary interventions to support patients with acute, sub-acute and exacerbations of long-term conditions.

Job responsibilities
1. Main Duties and Responsibilities

• Undertake comprehensive holistic assessments including physical, psychological, social and environmental factors.
• Complete risk assessments.
• Develop, implement and evaluate patient-centred treatment plans.
• Recognise deteriorating patients and escalate appropriately.
• Support patients with acute and chronic health conditions in community settings.
• Support admission avoidance and facilitate safe discharge from acute services.
• Deliver clinical interventions within scope of practice including:
• Venepuncture
• Catheter management
• Wound management
• ECG recording
• Facilitate Community Support workers to complete Medicines management
• Monitoring of Acute and/or long-term conditions
• Participate in daily virtual ward reviews and MDT meetings.
• Ensure accurate and contemporaneous documentation.

You will use evidence-based principles/current best practice and specialist skills to assess, plan, implement and evaluate interventions and to ensure that all members of your team do so accordingly.

Your caseload management and communication skills will enable you to prioritise patient care and manage your own caseload. You will also work closely with senior clinicians, social workers and other health and social care professionals to establish and maintain a whole systems approach for our patients
