Crane Valley Primary Care Network is looking for a motivated and skilled Clinical Pharmacist to join our pharmacy team. This is an excellent opportunity for a pharmacist who is passionate about improving patient care, optimising medicines use, and supporting the delivery of high-quality healthcare services across our practices.
Working as part of the PCN team, you will provide clinical pharmacy expertise to patients and healthcare professionals, supporting safe, effective, and evidence-based prescribing.
Main duties of the jobYou will be responsible for:- Conducting structured medication reviews and supporting patients with long-term conditions
- Reviewing and optimising medicines to improve patient outcomes and safety
- Managing medication-related queries from patients, GPs, nurses, and wider healthcare teams
- Completing clinical medication tasks, including prescription queries and medicines reconciliation
- Providing advice on prescribing, medicines safety, and appropriate use of medications
- Identifying opportunities to improve prescribing quality and reduce medicines-related risks
- Supporting audits, quality improvement projects, and medicines optimisation initiatives
- Working collaboratively with GPs, practice teams, community pharmacy, and other healthcare professionals
- Acting as a clinical resource for medication-related advice across the PCN
We are looking for a pharmacist who has:- Registration with the General Pharmaceutical Council (GPhC)
- Relevant primary care or clinical pharmacy experience
- Strong clinical assessment and decision-making skills
- Excellent communication and organisational skills
- A proactive approach to medicines optimisation and patient-centred care
- The ability to work independently while contributing effectively within a multidisciplinary team
Experience within general practice, PCNs, or NHS primary care services is desirable.
