We currently have an exciting opportunity for an enthusiastic and forward-thinking Registered Nurse who is looking to enhance and develop their career in the primary care setting. We need you to help us develop a proactive approach to population health management to identify people who require quality care to manage the impact of their condition. We offer a structured holistic assessment to those identified to be most at risk.
You will be working as an autonomous practitioner as well as collaboratively with the wider integrated frailty service offered by our community colleagues. You will work as part of our Frailty Service within our Wellbeing Team and other practice staff to make this a key role in delivering care to this patient cohort.

Main duties of the jobTo assess, plan, implement and evaluate care plans for patients; promoting proactive independence and autonomy; working within a multi-disciplinary team.
To build and continually advance own clinical knowledge, skill and competence based on current evidence through educational programmes.
Provide specialist advice around the management of fragility to others.
To work in collaboration with other professionals within our neighbourhood to deliver and ensure proactive care is provided appropriately to our population
To demonstrate clinical effectiveness of service delivery by use of evidence-based practice and outcome measures.
To have the skills to take a history, assess, examine, diagnose, and develop a management plan.
To recognise, assess and manage risk across the immediate and wider working environment and make appropriate decisions autonomously ensuring statutory requirements are met.
To be responsible for patient safety through knowledge of systems, legal requirements and understanding of litigation.
To communicate effectively verbally and in written form in the exchange of highly complex, sensitive, or contentious information in demanding situations using de-escalation, mediation, resolution, and professional Duty of Candour.
To evaluate care, taking appropriate action leading to improvement in quality standards through clinical audit, root cause analysis and dealing with complaints.

About usWe are located in a beautiful part of North Dorset with excellent links both road and rail with London. There are fantastic local schools both state and private. The coast is within easy reach for sailing, diving and outdoor pursuits.
Youll be based across our Blackmore Vale Partnership surgeries (Sturminster Newton, Shaftesbury & Marnhull). Hours are Monday to Friday during the day, and a full driving licence is needed.

Person SpecificationADDITIONAL REQUIREMENTSEssential- Demonstrable skills in written and spoken English to a standard which enables the post holder to conduct the full range of duties and responsibilities of the role effectively.
QualificationsEssential- Registered practitioner with further professional qualifications or demonstrable extensive experience in the relevant specialty
