We are seeking an experienced and motivated Advanced Nurse Practitioner to join our friendly, ambitious and supportive multidisciplinary team. This is an exciting opportunity to play a key role in delivering high-quality patient care whilst helping shape the future of healthcare within our community.
As a valued member of our clinical team, you will work alongside GPs, Practice Nurses, Clinical Pharmacists, Paramedics, Healthcare Assistants and our wider Primary Care Network colleagues to provide outstanding care to our patients.
Main duties of the jobThe RoleAs an Advanced Nurse Practitioner, you will work autonomously and collaboratively within the practice team, providing expert assessment, diagnosis and treatment for patients presenting with acute and chronic conditions.
Responsibilities include:- Assessing, diagnosing and treating patients with undifferentiated and complex presentations
- Managing acute and long-term conditions
- Independent prescribing in line with evidence-based practice
- Same-day and urgent care appointments
- Home visits where appropriate
- Providing clinical leadership and contributing to service development
- Supporting quality improvement and clinical governance activities
- Working closely with the wider multidisciplinary team and community services

Job Summary
The Advanced Nurse Practitioner is an autonomous practitioner responsible for the assessment diagnosis treatment and management of patients presenting with acute illness minor illness long term conditions and complex healthcare needs.
The postholder will work independently and collaboratively with the wider multidisciplinary team to deliver evidence based care and improve patient outcomes. The role includes clinical leadership service development health promotion education and quality improvement activities.
Main Duties and ResponsibilitiesClinical Responsibilities
- Assess diagnose and treat patients presenting with acute and chronic conditions.
- Undertake comprehensive clinical assessment and examination.
- Formulate differential diagnoses and management plans.
- Independently prescribe within scope of professional practice.
- Manage patients with long term conditions.
- Provide same day and urgent care appointments.
- Undertake home visits where appropriate.
- Refer patients to secondary care and other services when clinically indicated.
- Request and interpret diagnostic investigations.
- Deliver evidence based patient centred care.
- Participate in vaccination and immunisation programmes.
- Support health promotion and disease prevention initiatives.
- Provide advice and education to patients and carers.
- Maintain accurate and contemporaneous clinical records.
- Participate in safeguarding processes and act in accordance with local policies.

- Provide clinical leadership within the nursing team.
- Support service development and innovation.
- Participate in clinical audit and quality improvement activities.

Person SpecificationQualificationsEssential- Registered Nurse with current NMC registration
- Advanced Nurse Practitioner qualification
- Independent Prescriber qualification
- Master's degree in Advanced Clinical Practice or equivalent experience
- Evidence of advanced clinical assessment and diagnostic skills
- Evidence of continuing professional development
Desirable- MSc in Advanced Clinical Practice
