Job summaryWe are an open, friendly, relaxed but motivated dispensing practice with a practice population of 13,250 operating over three sites in mid-Cornwall. We serve the most deprived population in the county. Despite the challenges this brings, we consistently receive very positive patient feedback and take pride in the strong relationships we have with our community.
We pride ourselves on being a collaborative practice, open to new ideas for quality improvement and different ways of working. We actively encourage colleagues to contribute to developing and refining how we deliver care.
Previous general practice experience is not essential. We welcome applications from nurses with diabetes experience in other settings (e.g. community, secondary care, outpatient clinics) who are keen to develop their skills in general practice. We will provide a structured induction and support from our experienced GPs and nursing team.
If you are a registered nurse with a passion for delivering excellent diabetes care and would like to help shape and develop our service, we would love to hear from you.
Main duties of the jobYou will work solely within diabetes care, supporting our existing team to deliver high-quality, evidence-based management for our patients with diabetes. This is an excellent opportunity for someone who is:
- Passionate about diabetes care and improving patient outcomes
- Innovative, with ideas for service development and quality improvement
- Keen to work as part of a supportive multidisciplinary team
You will support our patients with Type 1 and Type 2 diabetes. You will be involved in diabetes reviews, patient education, diabetic medication reviews and adherence, and contributing to the ongoing development of our diabetes service. Knowledge on how to initiate insulin is not essential.
You will work alongside other members of the diabetic team and wider practice team to ensure a proactive, patient-centred approach to diabetes management, focusing on prevention, treatment optimisation, and self-management support.
About usThe practice serves a population of 13,500 patients over 3 sites in mid-Cornwall. The 3 sites are all dispensing practices, within close proximity and within approximately 5 minutes of the A30, providing convenient road access from many parts of Cornwall. Two of our sites are located in villages on the Mid Cornwall Metro rail line.
Our main site in Roche is in modern premises and we are co-located with Arbennek District Nursing Team, which enables us to have a very close working relationship.
The practice clinical staff are made up of a broad MDT with 7 GPs, registrar, two paramedics, an ACP, a paediatric nurse, clinical pharmacists, pharmacy techs, 4 practice nurses, 4 HCAs, 2 FCPs, an IPS worker, workflow admin, 2 care coordinators and a social prescriber.
We are an active teaching practice hosting a GP registrar, and nursing and paramedic students. We hold regular clinical meetings and personal development is highly encouraged.
The practice are consistently high QOF achievers, with consistently high positive patient feedback.
We are proud to be active members of both the well-regarded Arbennek PCN and the Cornwall Deep End Network. Through both of these partnerships and alongside the INT we have been able to develop innovative initiatives such as Community Hubs that help address health inequalities and improve opportunities for our patients.
